Days & Hours: Monday-Friday 730am-4pm Key Responsibilities: SENIOR/LEAD & Oversight (Multi-Site) Serve as the front desk team lead across assigned locations. Ensure consistent front desk workflows, processes, and service standards across all sites. Partner with clinic directors to review performance and identify training needs, that need to be addressed. Escalate behavioral concerns or unresolved patient/staff issues to leadership/administrator. Provide low-level resolution of patient complaints; escalate when necessary. Staffing & Coverage Ensure adequate front desk coverage is maintained at all times. Flex personal schedule to assist with last-minute coverage due to PTO or sick leave. Maintain and do FD functions and duties for approx 30 hrs a week. Training & Development Train and onboard new front desk hires as assigned. Conduct weekly 1:1 check-ins with front desk team members at each site. Promote standardization of training, workflows, and best practices across sites. Administrative Duties ( Approximately 10 hours/week) Review and submit timecards weekly (Fridays) for front desk staff. Hold weekly touchpoints (30–60 minutes) with assigned regional leader for updates and support. Coordinate regular feedback sessions with clinic leadership on front desk performance. Address operational questions and provide on-the-ground support throughout the week. Requirements: • High school diploma or GED is required., • One (1) year of medical office experience preferred, with familiarity in EMR systems a plus., • One year of experience in a customer service-oriented role is strongly preferred., • Proficiency in Google Suite is required., • Strong communication skills and the ability to interact professionally with customers, co-workers, and management, while calmly and effectively resolving client concerns, • Ability to work effectively within a team environment., • Excellent interpersonal skills and the ability to quickly adapt to new programs., • Ability to successfully complete in-service training., • Ability to observe, evaluate, and record patients' conditions, reactions, and changes in physical condition., • Ability to maintain a professional attitude and conduct in the welfare of patients., • Strong record-keeping and report-writing skills., • Ability to use logic and problem-solving skills to resolve issues, • Ability to work independently under tight deadlines in a rapidly changing environment, • Excellent time management and organizational skills Minimum 2 years of front desk or administrative experience in a healthcare or multi-site clinic setting. Job Description JOB DEFINITION: The Psychiatric Rehabilitation Care Coordinator (PRCC) functions as a member of a multidisciplinary treatment team to provide skill-building, vocational, and care-coordination services to individuals diagnosed with mental health and/ or substance use disorders. The PRCC partners with clients to identify social, community, and skills-based needs, and works with individuals to develop skills needed for meeting those needs. This role directly serves clients, engages with clinical treatment team members, and advocates with community-service agencies to reduce reliance on the behavioral health system, increase wellness self-management, and promote recovery. EDUCATIONAL REQUIREMENTS: Bachelor's Degree in one of the following fields: Social Work, Psychology, Education, Rehabilitation, Occupational Therapy, Counseling, Community Mental Health, Sociology, Speech and Hearing, Physical or Recreational therapy. Experience with community-based services, including vocational supports and linkage to available benefits & resources essential to independent living is preferred. DUTIES AND RESPONSIBILITIES: • Assists in the evaluation process and in identifying a problem list, short-term goals, and long-term goals to be incorporated in clients' treatment plans., • Participates in the development/documentation/review and update of client centered treatment plan in consultation with clinical team members to ensure focus on desired goals., • Provide psychiatric rehabilitation and case management services in individual and/ or group formats to individuals and supportive family members in various locations, including in-office, in-community, in-home, and via telehealth, based on client needs., • Provides telephonic as well as face-to-face outreach, engagement and service planning in the field., • Helps client develop or regain physical or mental functioning or adjust to disabilities by implementing programs involving prevocational, vocational, education, independent living skills, activities of daily living, educational, recreational, and social activities., • Acts as a linkage to community services including medical, behavioral, residential, entitlement and any other needed services per interdisciplinary care plan., • Promotes maximum independence by selecting and constructing therapies, • Maintains patient records, statistics, and other related reports that reflect patient treatment in an organized, timely and accurate manner (i.e., evaluation, daily treatment record, physician update, discharge summaries, collateral contacts) within the scope of staff education and licensure., • Acts as a resource to team members in the areas of rehabilitation, care coordination, and linkage to community resources., • Attends and participates in multidisciplinary team meetings and internal/ external trainings to provide input/feedback, review client status, update plans and goals, and review outcomes., • Participate in ongoing supervision, training/education and internal/external meetings as, • Facilitate sharing of data with the individuals to whom it applies to facilitate partnered decision-making and to keep clients informed of progress, • Complete QI Training during onboarding and participate in QI refresher training as needed., • Ability to cultivate a culture of inclusion for all employees that respects their individual strengths, views and experiences.
