Job Description Client Care Coordinator Supervisor PRIMARY FUNCTION/PURPOSE: Supervise the delivery of services provided by Client Care Coordinators (Social Workers) to enhance existing services to homeless families. This includes the completion of a thorough assessment, and the development and completion of a bio-psychosocial for each family upon intake, crisis intervention, counseling, client advocacy, and referrals to government and/or community-based resources. If needed, this individual will ensure that recommendations of achievable goals be implemented on each family’s service plan, leading to permanent housing, and strengthening the overall permanency outcome for clients in shelter. MINIMUM QUALIFICATIONS: • Must be a Licensed Clinical Social Worker (LCSW) in the state of New York upon hire or within three (3) months of hire, • Minimum of 3-5 years of experience in social services, counseling, crisis intervention, family services, preventive services, housing services, or homeless services, • At least two years of supervisory experience in social services, counseling, crisis intervention, family services, preventive services, housing services, or homeless services, • Knowledge in three or more of the following areas:, • Experience in supervision of social workers, and written communication skills coupled with strong interpersonal skills, • Proficient in MS Office, Outlook, Excel, PowerPoint, and Adobe PRIMARY RESPONSIBILITIES: • Manage and supervise a team of Client Care Coordinators to develop a team of skilled professional staff, including recruitment, training, supervision, evaluation, and staff scheduling to achieve the objectives of both CRF and Agency Funders, • Guide client care coordinators in the development, implementation, and achievement of program goals, • Handle administrative, operational, and programmatic issues that might affect completion of comprehensive bio psychosocial assessments for each client to understand strengths and service needs, • Work effectively as a part of a multi-disciplinary team to enhance client engagement and the delivery of quality service, • Enhance the delivery and coordination of mental health and related services to clients in shelter, • Ensure that Client Care Coordinator reports are clearly written and capture assessment findings and recommendations, • Confer and consult with professional and technical personnel in implementing a multidisciplinary approach to client care and well-being, • Ensure that Client Care Coordinators are providing short-term counseling, outreach services, crisis intervention, risk assessment, safety planning, and psycho-education for clients, • Collaborate with ACS and/or prevention services agencies, and participate in conferences to advocate on behalf of clients as needed, • Ensure that Client Care Coordinators are providing clients with appropriate referrals and linkages between mental health providers, government agencies, and other related community-based services as needed, • Serve as agency/program liaison between DHS, government agencies, community-based organizations and/or groups, • Develop and Implement group work to be facilitated by Client Care Coordinators to address family issues, enhance life skills etc., • Ensure that Client Care Coordinators are completing all documentation in a timely manner, • Ensure that Client Care Coordinators assess each client/family for supportive housing, complete supportive housing applications when applicable, and accompany residents to supportive housing interviews when needed, • Ensure that Client Care Coordinators complete and discuss exit summaries with each client, and provide clients/families with referrals and supportive services identified in their new community, • Participate in family meetings with case work and housing teams to discuss planning efforts and progress, • Networking and establishing successful linkages with government and community-based organizations to create resources for both families and staff, • Work in collaboration with the Director of Social Services and the Housing Coordinator to ensure that all services lead to permanent housing placement, • Complete special projects and assignments as needed or identified by leadership, • Must work one late a week from 11 am -7 pm to accommodate both staff and families, • Provide on-call crisis intervention support to Client Care Coordinators, • Perform other duties as assigned CLIENT Having a number of years experience in PR and comms, the Account Manager / Senior Account Manager will bring an aptitude for working with media to co-create strategic, creative stories with junior team members. SourceCode employees at this level are responsible for providing senior counsel to our clients, leading long-term strategy, and creative campaign ideation and execution. Schedule may vary due to program needs. JOB SUMMARY: Provide high-quality, culturally competent child development and program administrative services to Grand St. Settlement Early Childhood Programs. The Site Director manages teaching staff and comprehensive services for the successful operation of the program. For the start-up phase for 2 years, this will be an evolving position. Interim roles and responsibilities will be adjusted and this position requires an individual to be flexible. Job responsibilities will be revisited once the center-based site is up and running. ESSENTIAL DUTIES AND RESPONSIBILITIES: Under the direct supervision of the HS/ EHS Director, the Center Director shall be responsible for: ● Planning: Oversee and approve the preparation of daily lesson plans, including individual plans for each child including goal setting based on identified needs. Coordinates coverage and scheduling for center’s staff; Verifies time cards and schedules staffs’ vacation time, sick leave and compensatory time to ensure program coverage and effective program operation. ● Program Implementation: Implement daily lesson plans in response to children’s needs and interests incorporating observations, anecdotal record keeping, knowledge of early childhood development and the Developmental Continuum ● ERSEA –Must maintain 97% of enrollment at all times under ERSEA requirements .Ensure the program is meeting the needs and effectively including children who are dual language learners and children with disabilities and their families in all program services. ● Family Partnerships: Assist teaching staff in developing a system offering parents opportunities for enhancing and increasing their child observation skills. Provide support for prenatal and Mental Health services. ● Communication and Service Coordination: Lead regular team meetings and participate in case conferences, as necessary, to ensure service coordination across GSS Early Childhood Services ● Record Keeping and Reporting: Maintain filing, tracking and documentation systems for the early childhood education service area and ensure confidentiality of child and family records ● Ongoing Monitoring and Self-Assessment: Ensure compliance with Head Start Performance Standards relating to the education service area. Participates in Annual Self-Assessment, Program Information Report (PIR), CACFP reporting, Community Assessment, Federal Reviews and Policy Council Meetings. Assist with monitoring of classroom environments regularly using formal and informal observation, implement researched and reliable tools (Environmental Rating Scales – ITERS/ ECERS and CLASS) ● Human Resources and Supervision: Provide ongoing verbal and written feedback consistent with their individual development plans for performance improvement, ensure adherence to Head Start performance standards, and to plan and direct work. Collaborate and manage coaching implementation and professional developments for all staff. ● Facilities: Ensures that facilities, materials, and equipment are safe, appropriate, and conducive to learning and reflective of the different ages and stage of development of each child, including children with disabilities in accordance with Head Start Program Performance Standards and DOHMH Article 47 regulations ● Community Outreach: Recruit and develop partnerships with cultural institutes and the Department of Education to support wrap-around services.
