Job Description Care Management Support Coordinator II Remote (Hidalgo Area, Corpus Christi, or San Antonio, TX) Location: Remote (Must reside in Hidalgo area, Corpus Christi, or San Antonio, TX) Pay Rate: Starting at $18.00/hour Schedule: Monday-Friday | 8:00 AM - 5:00 PM (Overtime as needed) Dean's Professional Services is seeking a dedicated and detail-oriented Care Management Support Coordinator II to support a dynamic care management team. This fully remote role is ideal for individuals who are passionate about improving member outcomes, providing exceptional customer service, and supporting care coordination efforts in a managed care environment. In this role, you will serve as a key point of contact for members, providers, and internal teams-supporting outreach, care coordination, and administrative functions that ensure members receive timely, high-quality care and access to essential resources. Responsibilities • Conduct outbound outreach to members to support care plan follow-ups, scheduling, and ongoing education, • Respond to inbound calls and inquiries from members and providers, resolving issues in a timely and professional manner, • Assist members with connecting to health plan benefits, community resources, and programs addressing Social Determinants of Health (SDOH), • Support onboarding activities including welcome letters, program materials, and member communications, • Maintain accurate and compliant documentation of non-clinical member records in accordance with state and regulatory guidelines, • Coordinate services and scheduling to support continuity of care, • Serve as a liaison between members, providers, and care management teams to ensure a seamless experience, • Utilize knowledge of health plan resources and benefits to guide members effectively, • Ensure all activities align with company policies, procedures, and compliance standards, • Assist with additional administrative and care coordination duties as assigned Qualifications • High School Diploma or GED (required), • 1-2+ years of experience in a healthcare, customer service, or administrative role, • 2+ years of experience in managed care, or working with vulnerable populations (preferred), • Strong proficiency with Microsoft Office (Outlook, Word, Excel, Teams, SharePoint) and virtual communication tools (Zoom), • Excellent communication, organizational, and problem-solving skills, • Ability to work independently in a remote environment while managing multiple priorities, • Knowledge of healthcare benefits, community resources, and care coordination processes is a plus Why Join Dean's Professional Services? • Competitive pay with a comprehensive benefits package (medical, dental, vision, and 401(k)), • Fully remote role with a consistent weekday schedule supporting work-life balance, • Opportunity to make a meaningful impact by improving access to care and member outcomes, • Work within a collaborative, mission-driven, and supportive team environment Dean's Professional Services is a national, award-winning staffing firm.
