Job Description PRIMARY FUNCTION/PURPOSE The Social Services Supervisor is responsible for overseeing a team of Case Managers to ensure the support and resources necessary to promote self-sufficiency and independent living skills for families’ w/children residing in temporary shelter are being provided in accordance with CRF, DHS and OTDA standards. This shelter is located in Long Island City, NY. (Queens) PRIMARY RESPONSIBILITIES • Ensure document folder in CARES (VDP, Client rules and regulations, Crib acknowledgment, ACS Video, etc.) is completed and documented immediately upon client arrival, • Provide field orientation and on the job training to Case Managers, • Create and execute plans for meeting contracted ILP completion and placement goals, • Ensure all Case Managers complete mandated CRF, DHS, and OTDA tasks in accordance with prescribed timeframes and mandates, • Ensure Case Managers are providing consistent services in accordance CRF, DHS and OTDA policies and procedures, • Monitor Case Managers work activities via CARES reviews, physical chart reviews, and spreadsheets, • Conduct individual and group supervisions with Case Managers. Provide staff with documented feedback of performance, including areas of strength and areas in need of development, • Conduct performance evaluations of Case Managers as per CRF, DHS, and OTDA policies and professional standards, • Collaborate with Social Services team to develop service plans to address clients’ needs related to housing, substance abuse treatment, mental health and/or employment, etc, • Meet with Senior Management (PD and APD) for individual and group supervision, • Assist families with identifying appropriate housing resources and assist application process as needed, • Monitor demographic report to better guide Case Managers and ensure program compliance with CRF, DHS, and OTDA expectations, • Attend and moderate Case Conferences with Case Managers and challenging, noncompliant and/or housing ready clients, • Establish linkage with community service providers to obtain housing, health, and social services for clients, • Assist in facilitation of intake, discharge, transfers, and placement into permanency as needed, • Complete ILP’s and other Case Management related tasks as needed, • Generate and distribute reports (i.e., exit projection, quarterly projection, case summaries), • Support in the acquisition of housing vouchers and placements, • Deliver crisis management and intervention services to clients, • Support efforts to place families into permanent housing MINIMUM QUALIFICATIONS • Bachelor’s degree in Social Work required, • At least two (2) years of experience in a related field. Job Description Summary: The Criminal Court program provides a safe reception center and various services to assist survivors and victims of crime navigate through the Criminal Court. The program provides counseling, crisis intervention, case management services, advocacy, information and referrals and practical assistance to survivors and victims of crime with an overall goal of promoting survivor safety and reducing the impact of victimization. The Coordinator is responsible for providing holistic support services to victims of all crimes. This included counseling and crisis intervention to assess the needs of victims and to help with the development of safety plans to address immediate risks and concerns. This plan may include the provision of in-person or telephone counseling; emergency assistance such as money for transportation; assistance with an Office of Victim Services (OVS) application; providing information and referrals, and follow-up services; and providing assistance with obtaining an order of protection, restitution, and other criminal justice remedies. The Coordinator will advocate on the client’s behalf with public entitlement programs, schools, employers, civil legal attorneys, and other community-based organizations, and coordinate and collaborate with the OVS Vocational Rehabilitation Unit and the New York Civil Legal Network to ensure that victims are receiving a comprehensive response. Sixty one apartments, 51% of the units in the building, will be master leased by CAMBA and will house chronically homeless single adults and families who, the head of households, are diagnosed with chronic health conditions. The Housing Specialist will utilize a developmental approach to provide housing support to participants 17 years old and older. Supports will include developing housing leads and resources to help them locate housing; facilitating workshops to build their skills and increase knowledge related to living independently and transitioning into the community; advocating on behalf of youth with landlords and brokers to overcome any challenges to securing suitable housing; and assisting with securing resources to help with moving expenses. Each area of focus requires expertise to ensure the youth can access housing opportunities, be supported emotionally, and continue to be plugged into to a supportive network after locating housing. Major Duties: • Provides housing assistance to young people in foster care including NYCHA, Section 8, supportive housing, Housing Connect and other forms of housing., • Develops and maintains up to date information about housing issues, available subsidies, and programs., • Work with participants to develop and implement individualized goals based upon the specific service needs of the participant, and with a focus on moving into to a more independent living opportunity., • Collaborate with Property Manager to ensure all housing requirements are communicated to residents and maintained., • Develop and facilitate workshops to build skills and increase knowledge around living independently.
