Job DescriptionCompany Description The Office of Innovation & Technology (OIT) is the central IT agency for the City of Philadelphia headed by the Chief Information Officer (CIO). The Technical Support Specialist plays a major role in supporting the effective use of technology for the Philadelphia Police Department. Occupying a newly renovated facility with a state-of-the-art tier 3 data center, you will part of a technology-forward support team. In this role you will provide first- and second-level troubleshooting and technology-related responses in person, by phone and electronically. This role provides desktop computing support to 6,500 Police staff; assist in developing and implementing support policies that promote a sustainable infrastructure for the benefit of the entire staff; and contribute to needs analysis, planning, and implementation of software and systems. You will play a crucial part in maintaining operational continuity by alternating shifts every two weeks, switching between the 7 AM to 3 PM and 3 PM to 11 PM schedules. Your flexibility and commitment to maintaining a consistent level of productivity during both day and evening hours will be essential to the success of our team. Job Description Essential Functions Customer Relations: § Utilizing the service desk application, you will receive incident and information request calls from the customers for IT services, and service that call either by resolving on point of contact or passing the call to the most appropriate person or team for resolution. § Monitor the progress of the call from inception to resolution. § Keep customers informed of the progress of their queries. § Escalate calls to the Service Desk Manager using predefined procedures where the resolution. FaSST/Connections CTI Resource Coordinators engage individuals experiencing homelessness with a history or present struggle with mental health challenges and/or substance use. The Case Manager is responsible for identifying and coordinating resources to meet the individualized needs of program participants. This role ensures implementation of service and treatment plans, advocates for participants to access necessary services, and helps identify program needs and service gaps while recommending improvement strategies. The Case Manager works collaboratively with program participants, clinical teams, and community partners to support recovery, stability, and overall wellness. All responsibilities are performed in alignment with APIS and RHD organizational values. Team-Based Planning * Participate as a member of a multidisciplinary team to provide holistic, recovery-oriented services. * Assist in developing and reviewing recovery plans in collaboration with program participants, clinical staff, medical providers, and other stakeholders. * Build and maintain relationships with community organizations and service providers to connect participants with available resources. Direct Client Support * Provide and coordinate community-based services that support recovery plan goals. * Assist participants with accessing services such as housing resources, healthcare, employment, education, and community supports. * Support participants in building independence, community connections, and engagement in services. * Travel within the program’s service delivery area and transport participants to appointments when needed. Administrative Responsibilities * Complete required assessments, documentation, and service records according to agency, funder, licensing, and accreditation standards. * Maintain accurate and timely case documentation in electronic systems. * Participate in team meetings and case reviews. * Perform other duties as assigned. Education and Experience * Bachelor’s degree in Human Services or a related field preferred, or equivalent experience. * Intermediate computer proficiency. * One year of case management experience preferred. * Valid Driver's License held for a minimum of three years * Minimum two years of experience supporting individuals in: * Behavioral health * Addiction recovery * Homeless services * Behavioral Health Home (BHH) or IDD services. Job Description Starting Pay Range: $45,000.00 Position Type: FTE (37.5 hours/week) PURPOSE: Provide direct assistance to individuals with a behavioral health disorder, in gaining access to critical mental health, medical, social, educational, and other agencies or resources whose functions are to provide the support, training and assistance required for a stable, safe, and healthy life within their community. FUNCTIONS: Provide BCM services to a caseload that will not exceed 30 individuals, meeting minimum performance and productivity standards as identified by BCM Supervisor. • Provide proactive outreach to the individual in their home, school, and community and engage with, empower, and advocate for needs., • Conduct ongoing assessment and assist individual with identifying strengths and needs., • Develop a personal goal plan in collaboration with the individual and any identified supports and routinely monitor progress., • Provide psychoeducation on behavioral health diagnosis and symptoms; respond to questions or concerns about their course of treatment including potential side effects from prescribed psychotropic medication., • Provide linkage to identified resources with individual’s consent. This may include facilitating referrals for expanded mental health programming., • Assist in developing Activities of Daily Living (ADL) skills (e.g., developing a system for remembering to take medication as prescribed, navigating public transportation, budgeting, housekeeping, etc.), • Serve as a clinical liaison and coordinate care between all service providers and systems serving individual which may include but is not limited to: behavioral health, psychiatric, medical, dental, educational, and other evaluations and appointments., • As appropriate, assist clients with applying for MA and Social Security applications or troubleshooting other insurance barriers that impact access to care.
