We are seeking a licensed Occupational Therapist who thrives in autonomous practice, values clinical excellence, and understands the unique impact of therapy in the home setting. Position Summary The Occupational Therapist provides skilled occupational therapy services to homebound patients in accordance with the physicians plan of care, applicable state and federal regulations, and agency policies. Services focus on enabling patients to safely perform activities of daily living (ADLs), instrumental activities of daily living (IADLs), and functional mobility within their home environment. Essential Duties & Responsibilities The Occupational Therapist will: • Conduct comprehensive occupational therapy evaluations in the patients home, • Develop, implement, and revise individualized Plans of Care (POC) in collaboration with the physician, • Provide skilled interventions including:, • ADL and IADL retraining (bathing, dressing, toileting, feeding, home management), • Upper extremity strengthening and coordination, • Fine motor and cognitive-perceptual training, • Energy conservation and work simplification techniques, • Adaptive equipment assessment and training, • Home safety and environmental modification recommendations, • Assess patient progress and modify treatment plans based on clinical response, • Educate patients and caregivers on:, • Safe performance of daily tasks, • Use of adaptive equipment and assistive devices, • Strategies to maximize independence and prevent injury, • Complete accurate, timely, and compliant documentation in accordance with:, • Medicare Conditions of Participation, • Texas HHSC home health regulations, • Agency policies and procedures, • Participate in interdisciplinary communication and case conferences, • Coordinate care with physicians, nursing staff, and therapy team members, • Supervise Certified Occupational Therapy Assistants (COTAs), if applicable, per Texas regulations, • Follow infection control, safety, and HIPAA requirements Required: • Current Texas Occupational Therapist license in good standing, • Graduate of an accredited Occupational Therapy program, • Minimum 1 year of experience preferred (home health experience strongly preferred), • Knowledge of Medicare home health documentation and regulatory standards, • Valid Texas drivers license, reliable transportation, and auto insurance, • Current CPR/BLS certification, • Experience with OASIS documentation, • Familiarity with home health EMR systems, • Ability to travel to patient homes within assigned service area, • Ability to assist patients with transfers and functional tasks as needed, • Ability to lift, push, pull, bend, reach, and perform therapeutic activities, • Competitive per-visit or salaried compensation (based on role and experience), • Flexible scheduling and manageable caseloads, • Supportive leadership with clinical insight, • Opportunity to work with a growing, mission-driven Texas home health agency Applesoft Homecare Services LLC is an equal opportunity employer and complies with all applicable federal and Texas employment laws.
