This is a 3+ month contract assignment (possibility of extension or conversion) and a great way to start working with a top-tier healthcare organization! Job title: Home Health Physical Therapist Location: Puyallup, WA 98373 Duration: Contract Shift: Mon - Fri 8am - 4:30pm. Guaranteed Hours -36 Travel: Patient homes - Pierce County, WA Mileage Reimbursement: Yes (between patient visits). Specific Duties: • Evaluate patients’ functional status and physical therapy needs in compliance with CMS guidelines., • Develop and implement individualized treatment plans in collaboration with physicians., • Deliver evidence-based therapy to relieve pain, restore function, and improve mobility., • Monitor and document patient progress, reporting changes to physicians and nurse supervisors., • Educate patients, families, and caregivers on treatment plans, progress, and functional limitations., • Complete timely and accurate documentation, including evaluations, progress notes, and discharge summaries., • Travel to patient homes within the assigned service area and nearby locations as needed., • Participate in care coordination, team meetings, and case conferences., • Supervise Physical Therapist Assistants (PTAs) and Home Health Aides as applicable., • Maintain compliance with agency policies, state regulations, and professional standards of practice., • Active Physical Therapist license in Washington State (or compact license)., • Current CPR certification., • Valid driver’s license, auto insurance, and reliable transportation.
