Job DescriptionBenefits: • Bonus Potiental, • Flexible Scheduling, • 1:1 Care, • Referral Bonus, • Competitive salary, • Flexible schedule, • Paid time off, • Training & development Reports To: Clinical Supervisor Provides Supervision To: NA No supervision responsibilities Responsibilities • Provide companionship and, if authorized, transportation to clients according to the care plan., • Assist client with general Activities of Daily Living (ADLs) such as bathing, meals, and safe ambulation/transfers., • Report any changes in the clients condition to the supervisor as outlined in the care plan., • Document care provided and maintain accurate records for the Agency., • Work collaboratively with the clients care team and ensure effective communication., • Promote client safety by following infection control practices and reporting unsafe conditions., • Attend in-service programs to meet compliance and notify supervisors of any educational needs.Qualifications, • Current Certified Home Health Aide (CHHA) certification in the state of New Jersey., • Be at least eighteen (18) years of age., • Demonstrated ability to read, write, and follow a written care plan., • Good verbal, written, and interpersonal skills., • Licensed driver with automobile insured in accordance with state and/or Agency requirements and in good repair.
