Work Shifts • Time: 8:00 AM - 4:00 PM, • Days: Monday to Friday, • Facility: Trailer 4 at Rikers Island Duties & Responsibilities This position works under the direction of the Assistant Chief of Service, Medicine, exercises wide latitude of independent judgment and initiative in supervision of overall administration and management of the Medicine Department. The position assumes full responsibility and authority in areas delegated. May direct a group of services across multiple jail facilities. Responsibilities include: (Detailed Task) • Serve as the clinical director for Complex Care, providing clinical supervision to clinicians (1 NP, 1 RN) who supporting care delivery and coordination for the more medically complex patients in the jails., • This role encompasses the following responsibilities:, • Establishing clinical criteria for inclusion., • Defining the overarching vision and program objectives., • Attending weekly Complex Care rounds to ensure the effective delivery of care., • Consulting on Complex Care patients and offering clinical guidance and support to front-line providers within jail facilities., • Providing direct patient care at least 50 percent of schedule, • Serving as a clinical point of contact for colleagues from other departments as needed., • Delivering patient education related to complex diagnoses., • Ensuring that patients are assigned to medically appropriate housing areas on Rikers Island., • Providing clinical input and support for medically appropriate discharge plans for patients being discharged to the community, including necessary medications and follow-up appointments., • Ensure safe transitions and coordination of care between the jail and external facilities, including the following tasks:, • Obtaining and reviewing external medical records for comprehensive patient assessment., • Reviewing the daily hospital census of Correctional Health Services (CHS) patients., • Collaborating with the Complex Care Nurse Practitioner to schedule appropriate specialty appointments, track these appointments, and communicate care plans to all relevant parties within CHS to ensure proper execution., • Engaging with hospitalists and inpatient specialty teams when CHS patients are hospitalized and as they transition back to the jail or community., • Arranging expedited admissions to the hospital and ensuring the success of planned admissions for Complex Care patients., • Collaborate with other CHS teams regarding shared patients, with particular emphasis on those involved in Mental Health Discharge Planning and Re-entry and Transition Services., • Participate in clinical release work and support legal advocacy in concert with the Clinical Court Advocacy (CCA) team by:, • Acting as a resource to resolve clinical inquiries from CCA staff (social workers) and providing input regarding patient eligibility for CCA services., • Interfacing with legal representatives to furnish relevant clinical information pertinent to a patient’s legal case., • Composing letters that detail medical conditions for legal documentation., • Assuming on-call responsibilities for critically ill patients, ensuring timely communication with hospital staff, CHS leadership, and legal representatives regarding urgent cases., • Engage in the planning, development, and implementation of programs, policies, procedures, and evaluative monitoring techniques designed to fulfill the objectives established by CHS and Health and Hospitals Corporation (HHC) leadership. Minimum Qualifications 1. Master’s degree from an accredited college or university in Hospital or Health Care Administration, Public Health or a related discipline and five (5) years of high-level responsible experience in Hospital, Business, or Public Administration or a Health Care Administrator in a position of direct responsibility for operations of a major part or all of a, health care setting, including substantial exposure in meeting community health needs; or, 2. Bachelor’s degree from an accredited college or university in disciplines, as listed in “1” above; and six (6)years of high-level experience in areas as described in “1” above. Maintains internal department inventory control tracking system to assure receipt., • Manages the implementation of PYXIS Med station to ensure the appropriate utilization of automation and information technology., • Manages the integration and maintenance of automation and technology to assure safe and appropriate use of medications., • Plans, directs, supervises, coordinates, controls, and reviews work of subordinate and administrative personnel relating to assigned tasks and projects within the pharmacy department. Makes recommendations for promotion, discipline, termination, corrective training, or another personnel action., • Assists in the planning, development, evaluation and problem solving of data collection and analysis, • Arranges for the repair of equipment throughout the department through written and telephone contact with Materials Management. Arranges for repairs of fixed property through the Department of Corrections., • Manages completed controlled substances requisitions separated by building and type (regular controlled and methadone)., • Oversees that adequate inventory levels of supplies are maintained in the pharmacy office. When requested monitors specific supplies or medications in satellite pharmacies., • Assists in the development and preparation of financial goals and pharmacy budget based on System-wide initiatives and competitive business trends. Assists in making recommendations regarding staffing patterns. 1. Valid license and current registration to practice as a Pharmacist issued by the New York State Department of Education (NYSED); and, 2. Four (4) years of satisfactory full-time experience as a licensed practicing Pharmacist, of which at least two (2) years shall have been in an administrative, managerial or supervisory capacity in a Pharmacy in a hospital, health or medical facility. The Medspan program provides patient education and care coordination around HIV, hepatitis C, and associated conditions such as hepatitis B and sexually transmitted infections (STIs). Medspan team members support patients through medication adherence support, discharge planning, and connection to care services. The Medical Director works across the Correctional Health Service Medicine service, with MedSpan staff and with other interdisciplinary Correctional Health Services (CHS) staff in all facilities to provide, improve, and evaluate patient centered infectious disease care. Responsibilities include: (Detailed Task) Clinical Responsibilities: • Identify individuals admitted to correctional facilities with infectious disease (ID) needs through the utilization of electronic health records, report management systems, and various tracking tools., • Provide direct patient care to individuals experiencing acute or chronic infectious disease conditions, including but not limited to HIV, hepatitis C and B, sexually transmitted infections, tuberculosis, skin and soft tissue infections, wounds, and systemic bacterial processes., • Deliver clinical consultation and guidance related to infectious diseases to site medical directors (SMD), infection control teams, and clinical staff. Administrative Responsibilities: • Offer direct administrative and clinical supervision for the infectious disease service, encompassing ID clinicians and the Medspan team., • Oversee staffing requirements for the infectious disease service within the facility., • Direct the formulation and execution of workflows and protocols pertinent to the infectious disease service., • Organize and lead regularly scheduled rounds and meetings for the ID team., • Facilitate conferences and seminars with other clinical services and both CHS-based and external re-entry programs., • Provide supervisory clinical guidance to the SMD and staff of the communicable diseases unit (CDU)., • Oversee admission and discharge criteria for patients with confirmed or suspected communicable diseases within the CDU., • Lead patient tracking initiatives within the CDU, monitoring clinical conditions, medication regimens, and laboratory results., • Collaborate with and provide clinical consultation to the infection control team., • Engage in initiatives focused on system-wide education, quality improvement, and population health., • Direct projects, analytical efforts, patient tracking, and quality improvement initiatives for the infectious disease service., • Provide supervisory direction and guidance for the Medspan Program., • Oversee the creation and implementation of staffing, workflows, and protocols for the Medspan team., • Direct regularly scheduled rounds and meetings for the Medspan team., • Lead research initiatives, patient tracking, and quality improvement efforts for the Medspan team. Minimum Qualifications Applicants must have all of the following: 1. Graduation from a medical school approved by the Council on Medical Education and Hospitals of the American Medical Association. 2. Completion of an acceptable internship in an approved hospital., 3. A license to practice medicine in the State of New York., 4. Certification by American Board in field of specialization or a faculty appointment of Associate Professor or higher in a medical school recognized by the American Medical Association., 5. Eight (8) years of satisfactory full-time paid experience in field of specialization including at least three (3) years in a supervisory or administrative capacity.(Training and experience required for certification will be credited toward required specialized experience.) About the job We are looking for a Logistics Associate to join our growing team in New York. This role will work closely with the logistics associates, driver, and station manager. Responsibilities: • Collection/Delivery - Operates all assigned vehicles, single, tandem, or multi axle configurations, forklifts, etc. in accordance with established safety standards, proper vehicle operation techniques and specified time schedules. Acts as the face of Shengsheng for all shipment collections and deliveries and ensures that customer interactions are handled in a pro-active and professional manner at all times., • Packing - Prepares and packages of all specialist packaging and temperature controlled consignments of various temperature zones ensuring all mandatory procedures are adhered to. Re-packs and/or re-ice shipments where necessary and ensuring all mandatory procedures are adhered to., • Documentation - Ensures the correct documentation such as airway bill, delivery slips, and invoices are accurately completed and processed with the shipment. Be aware of the regulations covering the transportation, labelling and packaging of dangerous/hazardous substances and observe and comply with these regulations at all times. Completes necessary paperwork, including accurate logbooks, records of cargo and any other assigned record-keeping duties on a daily basis., • Loading/Unloading - Loads and unloads containers, pallets, or materials and products, safely on trucks, trailers, or railcars by hand or by forklift truck. When loading, ensures loads are evenly balanced and distributed from the front of the trailer to the back and from side to side, as well checks axle weights and loads them in accordance with the DOT regulations., • Warehouse - Performs all necessary duties to maintain a clean, tidy and safe warehouse, ensuring all materials are stored in the designated areas. Reports any shortfalls of packaging stock, to the appropriate person, to ensure minimal stock levels are always maintained. Ensures refridges, freezers and incubator/oven temperatures are checked and remain within prescribed parameters and maintain accurate records thereof. Requirements: Must-Have • GED degree, • Must hold a full, clean driving license, • Must have commercial van/truck driving experience, • Pharmaceutical delivery experience, • TSA Qualified, • Proven ability to work well under time pressure, • Demonstrates a passion for customer service, • Ability to check and maintain accurate documentation
