Overview Conducts comprehensive assessment of member UAS-NY for potential new members and existing members' conditions clinical, environmental, and social to establish an individual plan of care needed to maintain the member safe in the community. Identifies solutions that promote high quality and cost-effective health care services. Manages requests for services from providers, members, and care management team and renders clinical determinations in accordance with VNS Health Plans policies as well as applicable state and federal regulations. Works under general supervision. • Conduct face-to-face or telehealth UAS-NY assessments according to state guidelines, policies, procedures, and protocols, • Utilize clinical skills to assess and document all aspects of the potential members long-term community-based needs, • Communicate with members, families, providers, and other parties as needed to complete an accurate comprehensive assessment, • Utilizes VNS Health and state-approved assessment questionnaire, guidelines, and documentation as well as interviews with members, family, and care providers in decision-making, • Performs in-home assessment for members who have identified significant changes in condition since last in-home assessment; provides comprehensive review and determination of member’s needs, including completion of UAS assessment questionnaire, tasking tool, and a projected service plan. Visits include all areas serviced by VNS Health Plans including upstate and downstate counties, • Performs in-home assessment on members to determine the appropriate service plan, including completion of UAS assessment questionnaire, tasking tool, and a projected service plan. Visits include all areas serviced by VNS Health Plans, • Explains VNS Health Plan benefits, including an explanation of the member's handbook, • Ensures compliance with state and federal regulatory standards and VNS Health Plans policies and procedures, • Identifies opportunities for alternative care options and contributes to the development of a safe member centered service plan, • Consult with supervisor and others in overcoming barriers in meeting goals and objectives, • Maintains current knowledge of organizational or state-wide trends that affect member eligibility, • Coordinates with other departments, e.g. Care Management, Legal Affairs, Grievance and Appeals, Compliance, Membership Eligibility Unit, Quality as needed, • Participates in requests for out-of-network services when a member receives services outside of VNS Health Plans network services, • Keeps current with all health plan changes and updates through on-going training, coaching and educational materials, • Participates in special projects and performs other duties as assigned Qualifications Licenses and Certifications: Current license to practice as a Registered Professional Nurse in New York State required. Job Description Title of Position: Client Care Coordinator (LMHC) Department: Transitional Housing Reports to: LCSW, Clinical Director Pay Rate: $70,000 Annually Positions reporting to this position: None FLSA Status: Exempt Purpose of the Position The role of the LMSW/LMHC Client Care Coordinator within the social service team is to introduce and provide clinical services to individuals and families living in Win Transitional Housing sites. The Client Care Coordinator is responsible for engaging individuals and/or families in long term services within the community. The LMSW/LMHC is supervised by the LCSW/LMHC Clinical Supervisor. Job Description Title of Position: Case Manager Department: Transitional Housing Reports to: Social Service Supervisor FLSA Status: Non-Exempt Salary: $25.22 per hour Positions reporting to this position: None Purpose of the Position The Case Manager is responsible for case management services for clients in transitional housing. The Case Manager will have an assigned caseload and will be responsible for the successful integration of both the housing and employment goals of each client and ensure compliance with Win’s contractual requirements with outside Funders Responsibilities: • Conduct an initial assessment of the family within 48 hours of arrival to the residence and complete the Family Case Assessment Form, Initial Service Plan, and all initial intake documentation., • Initiate referrals for necessary emergency services., • Initiate/complete intake information (CARES data entry and supporting documentation) in the Uniform Client Chart., • Ensure CARES data and supporting documents are printed and filed in designated sections in the client’s chart., • Conduct orientation with clients and review the facility rules, clients’ rights and responsibilities, grievance/complaint process, emergency disaster procedures, etc., • Assist clients with transition to assigned residential unit and housekeeping items., • Provide comprehensive case management, including crisis intervention, counseling services and other appropriate support services., • Commit to using Evidence Based Practices (EBP); strength-based case management, motivational interviewing, trauma-informed care, critical time intervention, family-centered case management, boundary setting, and cultural-reflexivity to help clients achieve their goals., • Develop an Independent Living Plan (ILP) for each client, which will define the client’s goals (in conjunction with the transitional housing income building specialists, housing specialists, child care staff and recreational staff), and track progress against goals, make modifications, and ensure compliance with the ILP., • Provide hands-on training, coaching and support to clients, and help create opportunities for practicing skills that will enable clients to achieve their ILP goals., • Ensure that clients understand and work towards meeting their responsibilities as outlined in the Client Acknowledgment Responsibility Form (CARF)., • Maintain client data in the Uniform Client Chart., • Ensure the CARES data entry and supporting documentation is complete, accurate and current at all times., • Assist families in obtaining income and public benefits (i.e., Public Assistance; SSI, etc.)., • Establish and refer clients to appropriate medical and mental health services, education/job training programs, legal and advocacy services and other community resources as needed., • Must track referrals and follow-up each client’s progress., • Assess job readiness of clients, utilizing Standardized Assessment tools., • Perform timely and accurate input of all required client information in the Uniform Client Chart, ensure compliance with OTDA regulatory requirements and DHS contractual requirements.
