POSITION SUMMARY The Behavioral Health Care Manager provides high quality and effective care management to Titanium Healthcare members. The BHCM is responsible for care management: outreach and engagement to members, engaging members in skilled therapeutic interactions to promote health behaviors, coordination of care, resource linkages, working with other professionals and organizations in the community to ensure quality of care for members, seamless transitions of care, and facilitating proper care for members. WHERE YOULL WORK This position is hybrid. You will work from home and go into the field as needed in the West San Bernardino County region. Standard business hours are Monday through Friday from 8:30 am to 5:00 pm. WHAT YOULL DO • Support ECM members with behavioral health conditions, with particular attention to ECM members with SMI and/or SUD needs, through brief interventions and behavioral activation strategies, • Case manages members with behavioral health needs, majority Tier 1 and Tier 2 members, • Responsible for utilizing brief behavioral health therapeutic interventions as necessary to improve the members ability to manage their own health, • Responsible for brief crisis interventions and warm hand-offs to local crisis resources as needed, • Educate members and families about chronic medical and behavioral health conditions to improve health literacy and assists in health promotion, • Assist with the coordination of medical and behavioral health access issues with PCP offices, specialists, and ancillary services, • Advocate for members to receive the highest quality of care, in a timely manner, within IEHPs network, • Provide effective care management for assigned members, including care coordination, linkage to resources, support in transitions of care, facilitating member self-efficacy and self-management, and all other activities associated with high quality, evidenced-based care management, • Engage and collaborate with staff outside the BH department, or with those outside BH discipline, to provide integrated care to assigned members, • Responsible for assisting members with care coordination needs, • Conduct comprehensive, holistic assessment according to the scope of clinician-both telephonically as well as in person, • Aid RN CM in overall review of member Community Health Assessment (CHA) upon completion by another care team member, • Works with members to identify health/wellness goals and incorporates these goals into the members shared care plan that facilitate communication among members and providers, • Participate in inter/transdisciplinary care team meetings to share information, update, and inform care plan, • Coordinate with internal and external health partners to support member comprehensive care needs, • Responsible for ongoing engagement with members, both in-person and on the phone, in a manner that utilizes evidence-based approaches (such as Motivational Interviewing) that promotes collaboration between the member and his or her medical/behavioral team, as well as to increase the members sense of control over their whole health, • Engage in proactive, member-centered utilization and quality review of Behavioral Health and Care Management services by members, • Understand and abide by all departmental and companywide policies and procedures, • Comply with all safety and injury prevention policies and regulations, • Perform other duties as assigned or required WHO YOU ARE • Fluent in English (written and verbal), • Excellent verbal and written communication skills, including the ability to convey and exchange information in a clear and concise manner, • Adequate hearing and vision (with corrective devices if necessary) to conduct assessments and documentation, • Ability to identify problems and use logic and related information to develop and implement solutions, • Excellent organizational skills and attention to detail, • Ability to work independently and carry out assignments to completion within the parameters of established policies and procedures, • Ability to communicate clearly in-person, by phone, and electronically, • Frequent use of computers, keyboard, and handheld/mobile devices, • Competent with computers, email, virtual platforms, and Microsoft Office based programs, • Ability to type for extended periods, • Must be able to remain in a stationary position, • Ability to operate a vehicle and travel to meet with members around the community; attend meetings and events as required or requested, • Must be able to move around the office and/or travel throughout community, • Work may occur in homes, shelters, outdoor settings, hospitals, or community organizations, which may include exposure to pets, smoke, odors, clutter or unsanitary condition, and varying temperature conditions, • Ability to climb stairsor navigate uneven terrain in community and home environments, • Ability to lift, carry, push, or pull up to 2025 pounds(e.g., laptop bag, forms, mobile equipment), • Ability to bend, reach, and conduct in-person visits in non-traditional environments, • Commitment to maintaining patient confidentiality and adhering to ethical standards in healthcare practice, • Ability to maintain professionalism and safety in diverse environments WHAT YOULL NEED • Masters degree in social work or counseling, • Minimum 1-2 years of experience in social service setting, such as case management or community health, • Current and valid Drivers License, • Proof of auto insurance, • Distraction-free home workspace with a secure internet connection NICE TO HAVES • Experience using an EHR/EPM preferred;
