Job DescriptionBenefits: • Competitive salary, • Opportunity for advancement Job Summary Mid-Level Clinicians (Nurse Practitioners and Physician Assistants) provide comprehensive, evidence-based wound care services in accordance with company policies, physician collaboration agreements, and applicable state and federal regulations. The primary objective of this role is to safely treat, manage, and heal wounds, while acting as a strong advocate for the patient and maintaining the highest standards of clinical integrity. Mid-Level Clinicians serve as the primary clinical liaison between patients, care agencies, primary care providers, collaborating physicians, and the organization. Work schedules and responsibilities vary based on employment status (full-time, part-time, or per diem). Core Principles & Professional Standards • First duty is to do no harm, • Act as a patient advocate at all times, • Deliver ethical, safe, and evidence-based wound care, • Recognize limitations and escalate care appropriately beyond Standard of Care Clinical Care & Patient Management • Travel to assigned patient locations and see patients at least once weekly, or as clinically indicated, • Perform comprehensive patient assessments, including wound evaluation and classification, • Identify wound etiology and contributing medical and social factors, • Develop, implement, and update individualized wound treatment plans, • Perform and oversee infection control measures, • Initiate and manage Standard of Care and recognize when escalation is clinically indicated, • Prescribe antibiotics only, as clinically appropriate and within scope of practice, • Obtain, process, and coordinate PCR swabs when clinically indicated, • Recommend non-antibiotic medications or therapies to the patients PCP or agency when outside scope, • Complete all SOAP notes on the same day of service by 9:00 PM local time, • Respond to documentation clarifications and medical records requests in a timely manner, • Review and respond to:, • Company email, • Internal chat messages, • Other clinical communications daily, • Serve as a liaison between:, • Patients and caregivers, • Home health agencies, SNFs, or other facilities, • Primary Care Providers and specialists, • Collaborating physicians, • Internal clinical and administrative teams, • Coordinate care plans, treatment changes, referrals, and follow-up needs, • Practice under required physician collaboration or supervision agreements, as dictated by state law, • Escalate care to the collaborating physician, PCP, or specialist when:, • Wounds fail to progress under Standard of Care, • Infection, ischemia, or other complications are suspected, • Clinical findings exceed the clinicians scope of practice Compliance & Professional Responsibilities • Maintain an active, unrestricted Nurse Practitioner or Physician Assistant license, • Maintain current malpractice insurance as required, • Practice strictly within scope of licensure and state-specific regulations, • Controlled substances are not prescribed in this role, • Adhere to company policies, clinical pathways, and documentation standards, • Participate in required training, education, and quality improvement initiatives, • Accurately document mileage and visit activity using Timeero in accordance with company policy to support reimbursement and extended coverage requirements., • Provide timely notification of all planned and unplanned time off in accordance with company policy to ensure appropriate patient coverage and continuity of care. Work Schedule & Travel Requirements • Full-Time:, • MondayFriday, • 8:00 AM 5:00 PM, • Part-Time / Per Diem:, • Hours vary based on patient volume and operational needs, • Travel Requirement:, • Clinicians must be able and willing to travel up to at least 55 miles from their home of record to see assigned patients, • On-Call:, • No on-call responsibilities are required for this position Performance Expectations & Key Metrics Performance is evaluated based on, but not limited to: • Timeliness and completeness of documentation, • Responsiveness to medical records and internal communications, • Adherence to clinical protocols and escalation guidelines, • Appropriate prescribing and ordering practices, • Quality of care coordination and communication, • Wound healing progression and avoidance of preventable complications, • Compliance with licensure, collaboration, and regulatory requirements Required Qualifications • Active Nurse Practitioner or Physician Assistant license, • Ability to travel to patient locations as required, • Strong clinical judgment and decision-making skills, • Excellent communication and documentation skills, • Ability to work independently while collaborating with a multidisciplinary team Primary Goal of the Role To deliver safe, compliant, and effective wound care with the goal of healing wounds, preventing complications, and improving patient outcomes.
