Please be advised that the use of AI in applications are monitored, we remain watchful of candidates who misuse these tools to generate an application that doesn’t accurately reflect their skills.Job overviewAre you currently a B3 or B4 looking for part time oppurtunities as a B4 community falls support worker?Do you have a drivers license and would like to visit patients in their own homes and provide holistic assessment and care to patients who have fallen or at risk of falls?Do you have interest in faciliatating exercise classes and providing education to patients?Main duties of the jobTo respond quickly to all referrals for patients who have fallen or at risk of falling, register and triaging the referral in accordance to the KPITo visit patients in their own home or place of residence and complete a comprehensive, evidence-based falls assessment and identify, order any equipment, additional care or support required to enable the person to remain at home safely.To faciliatate and support the Luton Falls Team in delivering all aspects of the falls strength and balance classes/ group programme as necessary to include: patient reviews, exercise assessment, education and psychological support. To analyse facts and make judgements within own competency level. Detailed job description and main responsibilitiesTo work within the Falls Team to provide a thorough intervention service for people who have had a fall or are at risk of falling.To complete a comprehensive falls assessment and identify any equipment, care or support required to enable the patient to remain at home safely and avoid unnecessary hospital admission.To assist the therapy team in the day-to-day running of community based falls strength and balance group programmes.To lead the delivery of falls strength and balance classes across Luton, therefore there will be a requirement to travel locally between community group venues, clinics and patients own homes.To also make recommendations and onward referrals as appropriate.To work closely with Luton integrated community services, the ambulance service and other health, social care and voluntary organisations in Luton to reduce falls.To do coordinator of the day duties to support in triaging referrals from all services, answer tasks from system 1 queries and contacting patients/nok for any queries.Person specificationExperienceEssential criteria- Experience of working in the community- Time management, ability to organise and prioritise own work- Experience in faciliating exercise class- Experience in lone working- Experience in providing equipmentsDesirable criteria- Experience in SystmOne or other similar electronic patient recordsKnowledge and SkillsEssential criteria- Car owner- UK drivers licence- IT skills, use of microsoft word, excel and powerpointDesirable criteria- Providing education and adviceQualifications and TrainingEssential criteria- NVQ level 3 or Degree or equivalent, e.g. BSc,MSc or Diploma entry level qualification into the Physiotherapy profession.- Completion of application form legibly and clearlyDesirable criteria- Training about falls or related topicsOn April 1st 2026, Cambridgeshire Community Services NHS Trust and Norfolk Community Health and Care NHS Trust will come together to deliver services as a single Trust.
