Our model combines clinical excellence, technology, and a premium patient experience — bringing continuity, coordination, and compassion back into healthcare. \n We’re looking for an experienced Primary Care Nurse or Nurse Practitioner to join us as a Care Navigator — a pivotal role at the heart of our service. \n The Role \n This is not a traditional nursing role. \n As a Care Navigator, you’ll be the first point of contact for patients, combining clinical expertise with triage, coordination, and relationship-building. You’ll guide patients through their healthcare journey, ensuring they feel supported and understood at every step. \n You’ll work closely with GPs and senior clinicians, while helping shape how a modern, digitally enabled practice operates. \n Key Responsibilities \n Patient Care & Navigation \n\n • Act as primary contact, delivering a personalised patient experience\n, • Triage queries, assess risk, and escalate appropriately\n, • Coordinate appointments, referrals, tests, and follow-ups\n, • Support patients navigating NHS and private pathways\n, • Provide continuity, reassurance, and advocacy\n\n Clinical Coordination \n\n • Work alongside clinicians to deliver safe, joined-up care\n, • Escalate clinical concerns appropriately\n, • Maintain accurate records in line with CQC and GDPR\n\n Operational Development \n\n • Help design care pathways and service models\n, • Engage with and improve digital/AI-enabled systems\n, • Support workflow development in a growing startup\n\n What We’re Looking For \n Essential \n\n • Registered Nurse or Nurse Practitioner (or equivalent AHP)\n, • 5–10 years’ experience, ideally in primary care\n, • Strong triage and clinical assessment skills\n, • Patient-centred, empathetic approach\n, • Excellent communication and relationship-building ability\n, • Experience in NHS primary care and/or private settings\n, • Understanding of CQC, confidentiality, and GDPR\n, • Comfortable using digital tools\n, • Able to balance clinical and administrative work\n\n Personal Attributes \n\n • Warm, professional, and personable\n, • Proactive and highly organised\n, • Adaptable and resilient\n, • Motivated to improve patient experience and outcomes\n\n Why Join Us? \n\n • Be part of a new model of primary care focused on quality and continuity\n, • Work alongside experienced GPs and clinicians\n, • Shape how care is delivered in a growing organisation\n, • Flexible hybrid working\n, • Supportive, high-trust culture\n, • Competitive salary with potential equity opportunity\n\n
