You will be the primary, independent practitioner tasked with safely and efficiently delivering infusion therapy to our customers suffering from chronic diseases and acute medical illnesses. The Transportation Coordinator will work with the transportation coordinator to coordinate transportation services. The Transportation Coordinator will be responsible for the Housing and Emergency Financial Assistance components. Responsibilities is to provide case management and support services. Engaging and re-engaging clients, client assessments, monitoring client's goals, monitoring outcomes, developing an individualized housing plan, monitoring viral suppression. Making referrals as needed. Assist clients with emergency financial assistance for all client's urgent needs. This position is needed to obtain program intake data. The Transportation Coordinator will be responsible for all services provide transitional, short-term, or emergency housing assistance to enable a client or family to gain or maintain outpatient/ambulatory health services and treatment, including temporary assistance necessary to prevent homelessness and to gain or maintain access to medical care. Activities will include the development of an individualized housing plan, updated annually, to guide the client's linkage to permanent housing.
