May make appointments, including follow-up appointments for patients in a high customer service environment in an efficient and timely manner across the healthcare continuum including physician office visits, imaging, and lab post-visit and during patient outreach. Performs and documents in a timely and efficient manner patient outreach and call backs for messages received in pool, MyChart messaging and confirmation calls. May administer vaccines and documents appropriately and in accordance with established policies. May collect specimens and handles per establish protocols including proper labeling and safety handling. Assists physicians/providers with monitoring and reporting lab results. File all reports, labs, radiology reports and miscellaneous correspondence in EMR and attaches results to appropriate orders. May obtain prior authorization for medications, procedures, and testing. BASIC FUNCTIONS AND RESPONSIBILITIES * Conducts early screening to assist in the identification of students with special needs and talents, students who are over-aged and under-credited, and students with physical and emotional challenges that have impacted their learning; and makes referrals to appropriate services, and support programs or agencies. * Provides counseling to potential dropouts to identify alternative academic and social support program options. * Conducts interviews with students and families in preparation for making referrals to appropriate school placements. * Communicates with feeding schools, provides information on school offerings, and assist students and families in selecting an appropriate match. * Supports on-going orientation activities for new students and their parents, beginning in September and continuing throughout the school year. * Prepares and maintains appropriate records and reports, as required. * Assists students in evaluating their abilities and interests, strengths, and weaknesses through use of records, tests, inventories, observations, consultations, and interviews. * Assists Enrollment staff in interpretation and use of student records and tests; interprets same data for students and parents. * Updates and confirms student graduation eligibility following a review of student transcripts * Administers and interprets interest inventories and aptitude tests for students as they relate to educational and career planning; interprets achievement test results for students, parents and staff. * Inform students and parents about the NPS graduation requirements. * Complete transcript /graduation audits and inform students and parents of the outcome Consults with parents and administrators to bring about better adjustment of students. * Consults and cooperates with other supportive services personnel to inform student and school-wide goal development and intervention planning. * Participates in conferences conducted by the Student Support Team and Child Study Team. * Cooperates with community agencies involved with students, such agencies as Aspira, Essex County Welfare, Division of Youth and Family Services, and the New Jersey State Employment Services to ensure continuity of services. * Interviews new students and recommends appropriate class placement. * Reviews student transcripts to assess progress toward graduation and develop a plan for course completion. * Participates in professional learning committees, professional development workshops, in-service trainings, and administrative meetings to keep abreast of new trends and techniques in the field of guidance and youth development. * Perform other duties and tasks assigned by Supervisor. This position is responsible for providing specialized respiratory assessment and care commensurate with training, experience and competence to patients with respiratory disorders. Respiratory Care Therapists recommend treatment plans and implement care as directed by the patient's attending physician and hospital protocol. Respiratory Care Therapists may be assigned other tasks for which they have special training and/or background experience, in order to maximize team efficiency and economy. Specific Accountabilities: • Assesses respiratory patient in order to determine physical and emotional care needs and capabilities., • Makes adjustments to inpatient care regimen to assure individualized effectiveness according to the patient's needs and capabilities, through use of protocols or physician contact, as necessary, to assure appropriateness of care., • Educates patient and/or caregivers to provide understanding of physiologic processes affecting their condition, the lifestyle modifications necessary to maximize health, the appropriate use of equipment and medication utilized in their care., • Works in conjunction with physician to provide respiratory consultation to assure implementation of appropriate and effective care orders and protocols, and to obtain meaningful diagnostic information., • Reports all adverse effects to include signs and symptoms of respiratory dysfunction from therapies and medications given., • Ensures 50% attendance on an annual basis at the regularly scheduled staff meetings., • Maintains professional growth and development through seminars, workshops and professional affiliations to keep abreast of latest trends in field of expertise., • Ensures no injuries to self or others by following safe work practices and policies. This includes, but is not limited to: security and safety, understanding of MSDS, equipment, infection control, fire, disaster, safe lifting and body mechanics., • Ensures self-compliance with organization policies and procedures, as well as labor agreements., • Ensures the interface with team members and other support groups is conducted in a courteous and efficient manner conducive with the organization’s values., • Conducts self in a professional manner and ensures personal appearance meets the standards necessary to perform the job function while representing the organization., • Ensures that additional accountabilities as may be required by management be handled in a manner necessary to meet organizational standards.
