This role has two primary focuses: providing NP-level hospice care to enrolled pediatric patients and supporting a specialist pediatric palliative care consultation service. The vast majority of visits will take place in the patient’s home, with occasional visits to hospitalized pediatric patients as needed. Some visits may also be conducted via telehealth based on clinical appropriateness and patient needs. This role is ideal for a clinician with strong skills in advanced symptom management, family-centered communication, goals-of-care discussions, hospice eligibility review, and interdisciplinary teamwork.
