Job Description Position Description The Care Manager has overall day-to-day responsibility for coordinating the activities of the care team for clients with complex medical and/or psychiatric conditions and for facilitating clients’ access to the full range of medical and psychosocial services efficiently and effectively. The Care Manager is mainly responsible for coordinating medical care by receiving inpatient and ER admissions of targeted clients. In addition, the Care Manager is responsible for visiting clients during inpatient stays and participating actively in discharge planning and care transition activities. The position currently follows a hybrid schedule. Job Description Position Summary: Hamilton-Madison House (HMH) is the leading provider of behavioral health services for New York City’s Asian community and the second largest provider in the country. HMH serves nearly 1,000 community members through a comprehensive program model that understands and incorporates the strengths of individual cultures and addresses the stigma related to mental illness. Services range from psychological services; personalized recovery-oriented services for individuals with mental illness; recovery programming for addiction in six languages and seven dialects; supportive housing for people with mental health issues; and training and internship opportunities for graduate students in social work, mental health counseling, and psychology. HMH is collaborating with the Teacher’s College of Columbia University to embark on a research project to implement the Family Check-Up for Suicide Prevention (FCU-SP). The aim of the study is to deliver the FCU-SP for Asian American adolescents (ages 11 to 17) at risk for suicide and their families. As part of the team, you will deliver the FCU to Asian American at-risk youth and their families at Hamilton-Madison House, a community based behavioral health center located in the Chinatown and Lower East Side neighborhoods of Manhattan. Job Description THE ORGANIZATION The Center for Justice Innovation is a nonprofit organization dedicated to advancing community safety and racial justice. Since 1996, we’ve worked alongside communities, courts, and those most directly affected by the justice system to build stronger, healthier, and more equitable neighborhoods. With a team of over 900 staff and an annual budget of $130 million, the Center carries out its mission through three core strategies: • Operating Programs that pilot new ideas and address local challenges;, • Conducting original research to evaluate what works—and what doesn’t; and THE OPPORTUNITY Staten Island Justice Center (SIJC) seeks to re-engineer the experience of criminal court in Staten Island, New York, by providing judges and attorneys meaningful alternatives to bail, fines, and jail sentences. Operating out of Richmond County Criminal Court along with community-based offices, SIJC is a team of social service providers, court-based resource coordinators, mental health practitioners, compliance specialists, and others who seek to improve the quality of justice. Supervised Release offers an alternative to jail by providing pretrial supervision, case management, and voluntary social services to people charged with misdemeanor and felony offenses, and in doing so, uses an arrest as a window of opportunity to change the direction of a participant’s life, avoiding the harmful effects of incarceration. Program participants are monitored to ensure their appearance at court dates and mandatory programming, and receive referrals to services like job training, drug treatment, and mental health counseling. SIJC is seeking a Clinician, Supervised Release Program (SRP). Reporting to the Clinical Director, the Clinician, SRP will be licensed a social worker and mental health practitioner responsible for providing therapeutically-informed support to participants. This position will leverage exemplary clinical and interpersonal skills to provide clinical support to participants enrolled in the program. Specifically, the Clinician, SRP will facilitate CBT-informed sessions for all Tier 2 participants, provide short-term counseling for any participant identified as a strong candidate for mental health support, and external referrals to therapy, as appropriate. This position provides hours towards LCSW accreditation. The Clinician, SRP will not carry a standard SRP caseload. Rather, they will work closely with the case management and specialty in-house referral teams to case conference about participants' strengths and needs and coordinate tailored support as appropriate. POSITION SUMMARY: The Information and Referral (I&R) Specialist assists with CHASI's Community Food Pantry operations and conducts outreach to substance users to engage persons in services, provides a wide range of linkage & navigation services, provides individual education to reduce health-related harm from injection and other drug use, conducts sterile syringe exchange, trains on opioid overdose reversal, and delivers HIV and hepatitis C (HCV) counseling and testing. The I&R Specialist primarily works at the community access center to reach long-term and newer substance users, to partner with them as they set their own goals for improving their health and well-being, and to help them navigate service systems to achieve those goals. DUTIES & RESPONSIBILITIES: • Conduct group presentations to engage and inform substance users about harm reduction interventions., • Conduct harm reduction-oriented individual and group education encounters with substance users to reduce the transmission of blood-borne diseases, other infections, and drug overdose., • Enroll individuals in State-authorized syringe exchange program and provide exchanges that promote the consistent use of sterile syringes and safe collection of used syringes and provide safer injection counseling., • Distribute Naloxone kits to reverse opioid overdose and provide group and individual training on their correct use of Naloxone., • Conduct HIV and HCV counseling, testing, partner notification and linkage services, in accordance with standards set by NYS Department of Health/AIDS Institute and CHASI., • Deliver community-based education and risk reduction education and materials to targeted populations and the community., • Provide information, referral, navigation, and coordination assistance that links individuals to health and human services responsive to their individually expressed needs and interests., • Collaborate with harm reduction and community access staff on the identification of service provision, trends in local drug use, and service providers that are substance user friendly., • Support the food pantry staff and volunteers during operations when needed., • Complete accurate documentation service provision using hard copy tools and electronic data systems., • Track inventory of harm reduction program supplies., • Complete internal and external training to maintain knowledge and skills related to harm reduction, culturally competent service delivery, and other job-relevant topics., • Actively participate in staff meetings and other agency activities., • Encourage a sense of commitment, camaraderie, and accountability among team members by modeling these qualities., • Screen incoming phone calls and provide administrative support on a periodic basis., • Provide services on the Mobile Health Unit as assigned., • Perform all functions in alignment with CHASI’s Mission, Vision, and Core Values, • Other duties as assigned. Requirements: QUALIFICATIONS: • High school diploma or GED required., • Experience working with substance using persons and diverse populations strongly preferred., • Conversant with harm reduction practices, philosophy, and interventions., • Bilingual English/Spanish strongly preferred., • Ability to work effectively independently and as part of a team with demonstrated ability to contribute positively to a team-based work environment., • Demonstrated proficiency in the use of office computer applications, including Microsoft Word, Excel, and Outlook., • Strong oral and written communications skills; ability to manage communications tactfully and professionally with a high attention to detail and accuracy., • Driver's license and clean driving record preferred., • Must have flexible schedule and be able to work weekends. MAJOR FUNCTIONS: The Outreach Coordinator is responsible for the supervision and delivery of mental health, substance abuse counselling, intervention services, education services, and case management services. The Coordinator is an integral part of the clinical team. He/she is directly supervised by the Sr. Director. The Outreach Coordinator is in order to conduct groups at various outside programs. Provide supervision, training and orientation for all Peer Specialists. SPECIFIC DUTIES & RESPONSIBILITIES: • Organizing various outreach groups and marketing outreach services., • Act as an integral member of the Outreach Team., • Must document individual progress notes on a weekly basis, maintain all group notes and charts according to the DOHMH regulations., • Responsible for screening and recruiting clients within the NYS Psychiatric Hospitals, Office of Mental Health License Programs/Housing, Shelters, Treatment programs and NYC hospitals, • Maintain All files in appropriate, designated locations in the Outreach office to ensure confidentiality is maintained., • When client contact is off-site all client notes must be secured outside the Program’s office, it is important to ensure confidentiality is respected both verbally and in written form., • All group notes must be brought into the office and securely stored at the end of each day., • Conducted regularly scheduled groups with clients at various sites in a timely manner., • Conduct a minimal of one Community Outreach event or activity to promote program services., • Outreach will be conducted throughout the five boroughs including responding to request from other community-based providers, health fairs, college campuses and onsite community presentations., • Properly document and file case notes and other reports in accordance with program protocols., • Collect data and report on client satisfaction regarding groups., • Attend meetings and training functions Safeguard confidential information., • Prepare monthly reports for the Sr. Director., • Prepare monthly Levels of Service (LOS) Report through DOMHH website., • Ensure supervision for staff as per Odyssey House internal policies. We are seeking a full time Psychiatric Nurse Practitioner (PMHNP) to provide psychiatric evaluation, medication management, and clinical consultaion across both the OASAS 820 residential programs and the OASAS 822 outpatient program. Working in close collaboration with the Medical Director and multidisciplinary teams, the PMHNP supports integrated trauma-informed care for adults with substance use disorders and co-occurring mental health needs. RESPONSIBILITIES • Conduct comprehensive psychiatric evaluations, including history and mental status examinations, for residential and outpatient clients, • Diagnose psychiatric conditions and establish individualized treatment plans, • Provide ongoing psychopharmacological treatment and medication management, including MAT (e.g., buprenorphine, naltrexone) where appropriate, • Provide and/or oversee administration of long‑acting injectable medications as appropriate, • Monitor medication efficacy, adherence, and side effects, • Collaborate with residential and outpatient treatment teams to support integrated care, • Provide clinical psychiatric consultation to program staff, • Complete accurate and timely documentation in the Electronic Health Record (EHR)., • Consult the NYS Prescription Monitoring Program (IStop) when prescribing controlled substances., • Provide psychoeducation to clients regarding diagnoses, medications, and health-promoting behaviors, • Contribute to program development and clinical best practices across both levels of care, • Perform other duties as assigned by the Medical Director.
