Job Description Program Summary: The Youth ACT team (Assertive Community Treatment) provides mental health services to youth and families who are at risk of entering or returning to a higher level of care such as inpatient hospitalization or residential setting with the goal of maintaining safety and stabilization. The Youth ACT team works closely together to address mental health concerns of youth and the impact these concerns have on the family as a unit. Each member of the team provides individualized treatment through assessment, counseling, and clinical services to youth and families in a variety of settings such as in the home, and other community-based settings. Position Summary: We are seeking a compassionate and experienced Psychiatric Nurse Practitioner to join our Youth Assertive Community Treatment (ACT) team. This role is an invaluable resource and plays a crucial role on a multidisciplinary team. The ideal candidate has a passion for community and family work. The Psychiatric Nurse Practitioner provides psychiatric evaluations and medication management for youth and their families in community and home settings. Responsibilities include treatment planning, collaboration with therapists and community organizations, medication oversight, emergency assessments, and participation in case reviews and quality improvement initiatives. Job Description We are currently seeking Senior Gas Project Engineer(s) in a hybrid setting located somewhere in Central NY, Western NY, or Northwestern PA. POSITION OVERVIEW Under supervision of the Clinical Supervisor the CASAC Counselor will provide counseling services in integrated outpatient program. The CASAC Counselor provides comprehensive assessment services to clients who may have substance use and mental health barriers to employment by applying clinical judgment to determine appropriate treatment needs and level of care. This includes performing SUD diagnostic screening, assessment, evaluation, orientation, treatment planning, case management, counseling, intervention, crisis intervention, patient education, reporting and record keeping in both individual and group counseling settings. The CASAC Counselor is also responsible for referrals and coordination of care in consultation with other professionals.
