Job Description BronxWorks helps individuals and families improve their economic and social well-being. From toddlers to seniors, we feed, shelter, teach, and support our neighbors to build a stronger community. Our outreach team provides mobile case management services to a caseload of over 400 chronically street homeless adults, with the ultimate goal of ending their homelessness. The Clinical Coordinator directly supervises the team’s case management staff and works with the Program Director to administer and sustain the program. Job Description The Entertainment Community Fund (formerly The Actors Fund), a nationwide human services organization that helps all professionals in performing arts and entertainment, seeks a Social Worker to provide comprehensive and clinically focused case management and support services for clients of The Friedman Residence to help them maximize their capacity for independent living. This includes primary intensive case management/counseling for residents identified as having the most significant psychiatric and/or substance abuse issues. This is currently a hybrid position. RESPONSIBILITIES The Social Worker will manage clinical cases for a caseload of residents receiving HRA HASA Supportive Housing Services in addition to seniors, low income working or disabled residents identified as having significant mental health and/or substance abuse issues. The Social Worker will assist clients in maintaining medical, emotional and housing stability utilizing counseling interventions and appropriate referrals, and maintain a consistent schedule of face to face client contacts. This includes orientation, interventions & counseling to address conflict resolutions when roommate conflicts arise. The Social Worker will assist in the coordination of estate planning, including required legal documents such as power of attorney and living will; advocate for clients in obtaining and maintaining all eligible entitlements and services. Maintain confidential up-to-date records, logs, release forms, and charts for each resident and assure the confidentiality of all information. The Social Worker is responsible for maintaining documentation in assigned time frames. This position will conduct intake social service interviews for new applicants which include applicant interviews & assessments/evaluations for appropriateness for shared living, roommate interviews, and program orientation to support services, and conduct initial assessment interviews with new residents, documenting a plan of service, established client/SW goals, and a time frame for follow up and review. MINIMUM QUALIFICATIONS • Masters in Social Work from an accredited school. LMSW required or obtain LMSW within first six months of hire., • Minimum of 2 years post masters experience working with HIV population, seniors, and adults with mental health issues., • Strong verbal, communication and organization skills, • Demonstrates strong clinical and engagement skills., • Ability to establish and maintain positive working relationships with staff and other agencies., • Proficiency in Microsoft Office (Excel, Word, Outlook, PowerPoint)., • Ability to be self-motivated., • Work evenings on occasion. PREFERRED QUALITIES • Flexibility with working off-site for client advocacy, • Spanish speaking a plus, • Knowledge of entitlements a plus ABOUT THIS POSITION This is a full-time salaried position based in the Entertainment Community Fund’s New York Housing program, The Dorothy Ross Friedman Residence in midtown, Manhattan, New York City.
