Job Summary Rising Strong (RS) is a holistic, family-centered shelter program offering on-site outpatient drug treatment, mental health services and life skills coaching -- supporting the whole family staying together as a family unit while they begin to recover from substance misuse, heal from trauma and rebuild their lives. This position serves as the assigned RS operational team member supporting families residing at Rising Strongproviding advocacy, support and accountability for families during their stay in the programwith the goal of addressing social drivers of health and ensuring consistent communication and accountability throughout the familys residency. This position is also tasked with representing the RS Operations Team in cross-system meetings including CFTs and FTDMs, and designed to act as the primary communication conduit with each familys formal support network. Ability to work nights, weekends and alignment with all agency protocols required. Job Duties/Responsibilities To perform this job successfully, an individual must be able to satisfactorily perform each of the essential duties/responsibilities listed below. Family Advocacy and Support • Work in conjunction with the Partnership and Operations Coordinator to support intake transitions, provide resident tours and program orientation support, manage assigned caseloads of families, assist residents with internal and external resources during their time in the Rising Strong program, • Work in conjunction with the Staying Strong Coordinator to ensure families have a clear path for their successful completion of the Rising Strong program and access to housing supports before graduation., • Create and maintain case management files (either hard copy or via Salesforce), documenting case management services received, progress toward goals, and all applicable ROIs and program compliance documentation., • Assist RS families in developing a clear list of the goals they wish to achieve during their time at Rising Strong., • Provide weekly one-on-one meetings with the families to help them solve problems and develop enhanced life skills., • Assess the familys need for community resources to ensure safely and recovery-oriented services., • Provide and create a supportive environment for parents and caregivers by encouraging hope in, and control over, their own lives as well as their childrens lives., • Promote self-determination, empowerment, personal responsibility for recovery and wellness, and assist parent/caregivers to advocate for themselves and their children., • As applicable: coordinate transportation to and from events and all aspects of the event workflow., • Provide feedback, as needed, to assist residents with conflict resolution, parenting and interpersonal concerns and attainment of goals., • Utilize Motivational Interviewing techniques to ensure the cross-system plan of care maintains a focus on strengths, needs and capacities of the family., • Work collaboratively with Staying Strong Coordinator in facilitating access to Octavias Closet, an on-site clothing resource., • Integrate Catholic Charities-wide efforts of addressing social drivers of health in housing, ensuring all services are trauma informed, and implementing strategies of ensuring services and hiring practices are addressing race, equity and inclusion disparities. Accountably: • Support families in understanding and incorporating operational rules, structures and expectations., • Provide required reporting for alleged abuse or neglect, parental relapse with drugs or alcohol, safety plan not followed, new safety issues identified, severe behavioral incidents, running away, or situation requiring medical attention or hospitalization. Family and Program Representation • Support & attend scheduled graduation & celebration events., • Collaborate with Operations, CBHS, & WISe to promote a team culture in which decision-making in treatment and the cross-systems plan ensures the familys needs are addressed and are person-centered., • Act as a partner to the family and as a liaison with community providers, including school representatives, legal systems, and formal support systems., • Participate in CFT (Child and Family Team), FTDM (Family Team Decision Meetings, and any other resident related meetings as assigned., • Completes monthly reports, and ensures operational alignment between families, staff, and external partners., • Assist residents with preparing for court hearings, FTDM (family team decision meetings) and other dependency-related appointments., • Attend appointments to support families and advocate for the familys needs from an operational perspective., • Advocate for systemic change that improves the lives of those served at RS; represent Rising Strong in the community. Agency Protocols • Uses appropriate administrative, fiscal, physical, and technical safeguards to ensure theconfidentiality,integrity, and security of CCEW client protected health information (PHI), per regulations outlined in the Health Insurance Portability and Accountability Act of 1996 (HIPAA)., • Will regularly utilize all forms of communication (email, text, phone, etc.) and database practices necessary for this position and as directed by supervisor., • Accurately complete documentation per required timelines, utilizing sales force, incident reports, and touchpoints., • As a mandated reporter, follows all procedures outlined in agency policies and procedures to report to the proper agency when there is suspicion and/or confirmation that a child or adult has been a victim of abuse or neglect., • Adhere to the tenets of Catholic Social Teaching and Catholic Doctrine., • Performs as a team member to assure that productivity outcome measures are achieved., • Perform related functions as necessary to support the mission of Catholic Charitiesaligning with all agency policies, procedures and employee handbook mandates. Job Qualifications To perform this job successfully, an individual must meet the minimum qualifications listed below. These qualifications are representative of the knowledge, skill and/or ability required to perform this job. Education/Experience: To perform this job successfully, an individual must have a bachelors degree in a related field and a minimum two years of direct case management/ care coordination services or an equivalent combination of education and experience. Experience working with individuals in recovery and experience working with families involved in child welfare required. Knowledge of community resources; experience working with issues of homelessness, child welfare, poverty, and violence against marginalized groups preferred. Certificates/Licenses: Must possess a valid Driver License and ability to drive for work use. Successfully pass background check applicable to position. • Requires respirator medical evaluation questionnaire, fit testing, training, and use of respiratory protection as required by the WA Industrial Safety & Act standards. Workplace Environmental Conditions While performing the essential duties/responsibilities of this job, the employee will be: • Client Population may include persons involved in gangs, with substance abuse concerns, homelessness and/or tendencies towards violent behavior., • Noise Conditions: exposed during a shift to constant or intermittent sounds at moderate to loud levels of sound dependent upon the activities of the Shelter., • Exposure to Trauma: Employees of Catholic Charities Eastern Washington provide direct services to vulnerable community members including those who have behavioral health challenges and those who have been exposed to trauma., • Potential for exposure to illicit drugs and air-borne contaminants (proper PPE provided).
