Job Description Hospice & Palliative Care Registered Nurse (RN) Case Manager Job Summary: Faith Hospice & Palliative Care, a division of Faith Health Group is seeking a Registered Nurse (RN) Case Manager who will plan, coordinate and provide hospice & palliative care in accordance with the patient's plan of care. As a Registered Nurse (RN) Case Manager, you will provide compassionate and comprehensive nursing care to patients in their homes or facilities. The Registered Nurse (RN) Case Manager role will focus on pain management and symptom control while supporting patients' emotional and spiritual needs during their serious illness or end-of-life journey.
