Job Description Hospice & Palliative Care Registered Nurse (RN) Case Manager Job Summary: Faith Hospice & Palliative Care, a division of Faith Health Group is seeking a Registered Nurse (RN) Case Manager who will plan, coordinate and provide hospice & palliative care in accordance with the patient's plan of care. As a Registered Nurse (RN) Case Manager, you will provide compassionate and comprehensive nursing care to patients in their homes or facilities. The Registered Nurse (RN) Case Manager role will focus on pain management and symptom control while supporting patients' emotional and spiritual needs during their serious illness or end-of-life journey. You will work closely with an interdisciplinary team to ensure holistic care that respects the wishes and needs of each patient and their families. Key Responsibilities of the Hospice Registered Nurse (RN) Case Manager : • Coordinate hospice and palliative care by working with other team members as well as with the patients/family members to ensure needs are met., • Evaluate the effectiveness of services to the patient and family on an ongoing basis., • Perform evaluations for hospice care along with informational visits as needed., • See patients in the home and facility setting as needed. Provide palliative care to manage symptoms and ensure comfort., • Participate in IDG meetings, case conferences and QAPI activities and meetings., • Consult with the attending physician and Hospice Medical Director concerning changes in patients’ condition requiring order changes and/or changes in the plan of care., • Develop and implement the initial plan of care and work with other members of the IDG in completing and maintaining the comprehensive plan of care., • Provide education to patients/family members regarding the dying process, plan of care and offer guidance with healthcare decisions., • Provide dietary counseling, taking into account health issues and dietary changes at the end of life., • Supervise and evaluate the care given by the Hospice Aide at a minimum of once every 14 days and as needed., • Provide guidance and supervision to the LPN and supervises the LPN a minimum of every 30 days., • Perform death visits., • Coordinate with the care team to develop individualized care plans., • Advocate for patients’ end-of-life care in alignment with their wishes., • Provide emotional and psychological support to patients and their families, addressing any concerns or fears., • Refer patients and families to other services when needed, such as counseling or spiritual aid., • Support and back-up On-Call nurses including perform on-call duties per agency need., • Maintain accurate patient medical records., • Document care delivery, symptoms, the status of patients, and other aspects of the care plan., • Current RN license in New Jersey., • One year of RN experience. Hospice experience preferred., • Must have excellent assessment skills and problem solving skills., • Demonstrated knowledge of nursing practices, hospice and palliative care, and symptom management., • Compassionate, patient-focused approach with proven ability to build rapport with patients and their families., • Excellent documentation skills and professional judgment.
