Job Description The Mental Health Clinician provides clinical mental health services to all age patients/families in the Institute for Family Health. Job Description PRIMARY FUNCTION/PURPOSE: Supervise the delivery of services provided by Client Care Coordinators (Social Workers) to enhance existing services to homeless families. This includes the completion of a thorough assessment, and the development and completion of a bio-psychosocial for each family upon intake, crisis intervention, counseling, client advocacy, and referrals to government and/or community-based resources. If needed, this individual will ensure that recommendations of achievable goals be implemented on each family’s service plan, leading to permanent housing, and strengthening the overall permanency outcome for clients in shelter. MINIMUM QUALIFICATIONS: • Must be a Licensed Clinical Social Worker (LCSW) in the state of New York upon hire or within three (3) months of hire, • Minimum of 3-5 years of experience in social services, counseling, crisis intervention, family services, preventive services, housing services, or homeless services, • At least two years of supervisory experience in social services, counseling, crisis intervention, family services, preventive services, housing services, or homeless services, • Knowledge in three or more of the following areas:, • Experience in supervision of social workers, well-being, • Ensure that Client Care Coordinators are providing short-term counseling, outreach services, crisis intervention, risk assessment, safety planning, and psycho-education for clients, • Collaborate with ACS and/or prevention services agencies, and participate in conferences to advocate on behalf of clients as needed, • Ensure that Client Care Coordinators are providing clients with appropriate referrals and linkages between mental health providers, government agencies, and other related community-based services as needed, • Serve as agency/program liaison between DHS, government agencies, community-based organizations and/or groups, • Develop and Implement group work to be facilitated by Client Care Coordinators to address family issues, enhance life skills etc., • Ensure that Client Care Coordinators are completing all documentation in a timely manner, • Ensure that Client Care Coordinators assess each client/family for supportive housing, complete supportive housing applications when applicable, and accompany residents to supportive housing interviews when needed, • Ensure that Client Care Coordinators complete and discuss exit summaries with each client, and provide clients/families with referrals and supportive services identified in their new community, • Participate in family meetings with case work and housing teams to discuss planning efforts and progress, • Networking and establishing successful linkages with government and community-based organizations to create resources for both families and staff, • Work in collaboration with the Director of Social Services and the Housing Coordinator to ensure that all services lead to permanent housing placement, • Complete special projects and assignments as needed or identified by leadership, • Half of the residents will be referred by the New York City HIV/AIDS Services Administration (HASA) and the other half will be apartments for affordable to low-income households. The Clinical Supervisor will provide clinical and administrative supervision to members of the clinical team. The Felony ATI Court, which is led by Administrative Judge Ellen Biben, seeks to forge a new response to individuals charged with felonies by linking them to community-based social services. The Court is the first of its kind, providing an innovative model for other jurisdictions seeking to develop and implement meaningful post-conviction sentencing options for individuals facing felony charges. Reporting to the Clinical Director of Felony ATI, the Social Worker will be part of a team that provides neutral, third-party assessments and case management services to the Felony Alternative to Incarceration (ATI) Court. The Social Worker will be responsible for conducting independent clinical assessments, providing referrals to community-based services, offering comprehensive clinical case management, and monitoring participant compliance. They report to the Clinical Coordinator, or Director of Clinical and Social Services, while working closely with their Building Manager. A Social Worker I is an exempt employee who is required to have a Masters’ Degree. They are expected to be able to work autonomously while developing 1:1 relationships with the residents. This relationship will enable the Social Worker I to provide a comprehensive range of social work services to the residents and enable the residents to establish and maintain safe and independent lifestyles in the community. WSFSSH strongly supports M.S.W. level social workers obtaining their licensing. We offer study groups for staff who are preparing to take their exam. WSFSSH will also pay for a study course if needed, and pays for up to 2 licensing exams, as well as registration/licensing fees. Once a MSW staff member has obtained their LMSW, WSFSSH will cover reasonably priced ceu credits in order to meet licensing requirements. A Social Worker I is an exempt employee who is required to have a Masters’ Degree. They are expected to be able to work autonomously while developing 1:1 relationships with the residents. Job Description Overview Physician Affiliate Group of New York (PAGNY) is adding a Unit Chief to the Inpatient Psychiatry department at NYC Health + Hospitals/Jacobi. Jacobi, located in the Bronx, accommodates more than 325,000 outpatient visits, more than 100,000 emergency room visits and more than 20,000 inpatient admissions annually. Responsibilities The Unit Chief collaborates with a robust interdisciplinary treatment team, including nurses, psychologists, clinical pharmacists, social workers, licensed creative arts therapists, and peer counselors to oversee daily operations of an Adult Inpatient Behavioral Health Unit. Oversee Unit Clinical Structure • Establish and oversee interdisciplinary teams and team meetings, • Ensure that community meetings and other therapeutic groups take place as scheduled, • Participate in Behavioral Codes on the unit, providing leadership to the staff and patient community, • Work with all teams on the unit to monitor length of stay and appropriate dispositions, particularly ensuring that:, • Patients who are refusing medications are taken to court promptly, • Patients who have reached their 18th day are referred for state hospitalization for continuing treatment, • Patients who would benefit from AOT are referred promptly, • Other potential barriers to discharge are identified and solved for, • Work with the medical-consultation team ensure that medical problems are properly identified and managed, • Ensure that legal statuses are up to date and accurate, • Provide clinical consultation to teams on your unit, • Assist and supervise unit psychiatrists and nurse practitioners as needed, • Monitor the need for psychiatrist assessments and other components of applications of aftercare, in such applications for appropriate housing, residential placements and state referrals, • Participate in Performance Improvement and Quality Assurance activities, including chart reviews.
