Job Description TITLE OF IMMEDIATE SUPERVISOR: Health and Wellbeing Director DEPARTMENT: Resident Care STATUS: Non-Exempt/Hourly SUMMARY The Clinical Coordinator assists the Health and Wellbeing Director with administration and organization of the resident care department. Responsibilities include resident care transitions, clinical protocols, quality assurance, regulatory compliance and oversight of staff scheduling. The Clinical Coordinator ensures that policies and procedures established by Orchard are implemented and maintained consistently. QUALIFICATIONS AND EDUCATION REQUIREMENTS • High school diploma or GED, • Completion of a Professional Nursing Program and licensed to practice as a registered or licensed nurse is preferred or successful completion of a state approved nursing assistant training program and a state approved trained medication assistant program, • Current, active certification on the Georgia Nursing Assistant Registry and current certification in medication administration, • Must successfully clear a background check, TB skin test or x-ray, and health screening, • Current CPR certification, • Must have a valid driver's license, • Current certificate of training for successful completion of the five (5) hour initial fire safety training provided by the Rules and Regulations of the Georgia Safety Fire Commissioner PRINCIPLE DUTIES AND RESPONSIBILITIES • Assists the Health and Wellbeing Director with administration and organization of the resident care department, • Coordinates and supports Health and Wellbeing Director with Personalized Service Plans and family care meetings, • Liaison between the community and residents, families, medical professionals, pharmacy, and vendors regarding resident care matters- Refers matters outside of the scope of position to the Health and Wellbeing Director, • Manages Family Help Line for resident families' direct access to the Resident Care Department, • Ensures proper follow through and documentation for residents with a change in care needs, • Maintains wellness files according to Orchard policy, federal and state guidelines, • Reports to Health and Wellbeing Director to identify, review, discuss potential changes in service / medication levels, • Manages and coordinates resident care appointments, • Informs the Health and Wellbeing Director of any changes in medication or service level that may result in a higher tier level or potential move-out, • Ensures weights and vital signs are obtained monthly for each resident prior to completion of monthly wellness visit, • Contacts resident's attending physician when necessary and or upon family request, • Maintains wellness supplies for the community daily needs and emergency kits, • Oversees the monitoring of each resident's medication and treatment profile to ensure each medication and treatment is administered as ordered and documented accurately, • Monitors the Medication Room, med charts, treatment carts for neatness, cleanliness, availability of medications and expired medications, • Oversees nurse and care staffing scheduling are in accordance with the operational and budgetary guidelines, • Supports Health and Wellbeing Director with interviewing and onboarding new nurses, community supervisors, Certified Medication Assistants and Resident Assistants, • Supports in the facilitation of ongoing trainings and skills competencies of resident care staff, • Actively participates in risk management programs, • Balances team and individual responsibilities, exhibiting objectivity and openness to others' views, • Contributes to building a positive team spirit, • Identifies and resolves problems in a timely manner, gathering and analyzing information to develop alternative solutions both independently and with groups, • Manages challenging and / or emotional resident(s), family member(s), and employee situations, responding promptly to their request for service and assistance, while soliciting feedback to improve service insuring that commitments are met, • Focuses on solving conflict and not blaming, • Listens to others without interrupting and keeps emotions under control- Remains open to others' ideas and tries new things, • At times, provides assistance with assigned personal care tasks that meet resident physical and emotional personal needs consistent with the Orchard philosophy, • At times, administers prescribed medication as ordered by a physician to residents and maintain related medical records under the supervision of the RN COMPETENCIES • Works with integrity; Responsibilities include but are not limited to all resident care programs, quality assurance and regulatory compliance, assisting in the training and on-boarding of all care staff including nurses, community supervisors and resident care team members. Job Description POSITION: Certified Medication Assistant TITLE OF IMMEDIATE SUPERVISOR: Health and Wellbeing Director (HWD) DEPARTMENT: Resident Care STATUS: Non-Exempt / Hourly SUMMARY This position administers prescribed medications and treatments under the direct supervision of the Health and Wellbeing Director. In addition, this position will provide personal care service to the residents and perform related tasks at the community in accordance with the plan of care assigned by the Health and Wellbeing Director. QUALIFICATIONS AND EDUCATION REQUIREMENTS • High school diploma or GED, • Successful completion of a state approved nursing assistant training program and a state approved trained medication assistant program, • Good standing on the Georgia Certified Medication Aide Registry and have no record of being terminated for cause relating to the performance of medication aide tasks (Rule 111-8-63-.20 5a), • Current certification in medication administration. Participates in an annual CMA skills competency check which meets the requirements contained in the standardized clinical skills competency checklist used to certify medication aides. 5c. Successful demonstration of CMA skills during quarterly observations conducted by a licensed registered professional nurse or a pharmacist 5d. Follow state CMA protocols in order for the community to maintain compliance and successful completion of quarterly drug regimen reviews conducted by a licensed pharmacist 5f. Successfully completes an annual comprehensive clinical skills competency review. The skills competency checklist is administered by a Georgia-licensed registered nurse, pharmacist or physician. Job Description POSITION: Certified Medication Aide (CMA) TITLE OF IMMEDIATE SUPERVISOR: Health and Wellbeing Director (HWD) DEPARTMENT: Resident Care STATUS: Non-Exempt / Hourly SUMMARY This position administers prescribed medications and treatments under the direct supervision of the Health and Wellbeing Director. In addition, this position will provide personal care service to the residents and perform related tasks at the community in accordance with the plan of care assigned by the Health and Wellbeing Director. QUALIFICATIONS AND EDUCATION REQUIREMENTS • High school diploma or GED, • Successful completion of a state approved nursing assistant training program and a state approved trained medication assistant program, • Good standing on the Georgia Certified Medication Aide Registry and have no record of being terminated for cause relating to the performance of medication aide tasks (Rule 111-8-63-.20 5a), • Current certification in medication administration. Must be able to submit written competency test and skills check-off list prior to assuming position, • Must successfully clear a background check, TB skin test or x-ray, and health screening, • Current CPR certification, • Current certificate of training for successful completion of the five (5) hour initial fire safety training provided by the Rules and Regulations of the Georgia Safety Fire Commissioner PRINCIPLE DUTIES AND RESPONSIBILITIES • Administer physician ordered medication to resident's and maintain related medical records under the supervision of the Health and Wellbeing Director, • Verify identity of residents receiving medication and record name of drug, dosage, route, and time of administration on specified forms of records, • Presents medication to resident and observes ingestion or other application, or administers medication, using specified procedures, • Takes vital signs or observes residents to detect respond to specified types of medication and prepares report or notifies designed personnel of unexpected reactions, • Documents reasons prescribed drugs are not administered, • Adheres to medication administration policy, • May receive supply of ordered medications and apportion, mix, or assemble drugs for administration to resident, • May record and restock medication inventory 5b. Participates in an annual CMA skills competency check which meets the requirements contained in the standardized clinical skills competency checklist used to certify medication aides. 5c. Successful demonstration of CMA skills during quarterly observations conducted by a licensed registered professional nurse or a pharmacist 5d. Follow state CMA protocols in order for the community to maintain compliance and successful completion of quarterly drug regimen reviews conducted by a licensed pharmacist 5f. Successfully completes an annual comprehensive clinical skills competency review. The skills competency checklist is administered by a Georgia-licensed registered nurse, pharmacist or physician. Testing includes observation and verbal testing scenarios o Administer insulin, epinephrine, and B12 pursuant to physical direction and protocol o Administer medications via a metered dose inhaler o Conduct finger stick blood glucose testing following established protocol o Administer a commercially prepared disposable enema ordered by a physician o Assist residents in the supervisor of self-administration of medications • Provides bed bath, shower or tub bath as appropriate, • Provides appropriate care of skin, hair, nails, mouth and feet, • Provides good body alignment, positioning, and range of motion exercises for non-ambulatory residents, • Assists resident in and out of bed and with ambulation, • Assists resident with use of bedpan, urinal, commode and bathroom, • Assists with feeding and dressing resident, • Monitors and records temperature, pulse, respirations and blood pressure, as directed by the HWD. Significant changes in the residents' condition will be reported to the HWD immediately, • Changes simple dressings and ostomy bags, • Change Colostomy bags, • Assist with catheter cares, • Reminds and assists resident in taking pre-measured medications, • Maintains an accurate and complete record of care provided and documents observations appropriately, • Performs various housekeeping duties related to resident care, • Responsible for maintenance of electronic medical records, assuring accuracy, completeness and compliance with regulations, certification standards, legal and ethical standards, • Supports, instructs and educates the resident, family, and caregiver, • Provides personal care to the resident in a compassionate and understanding manner consistent with the Orchard philosophy, • Maintains the dignity of the resident, • Provides person-centered care across treatment settings to improve the quality of life, • Consults and collaborates with the interdisciplinary team and others involved in the resident's care, • Provides leadership oversight of team members and accountability to policies and procedures such as uniform, shift start times, absenteeism and professional interactions, • Focuses on solving conflict and not blaming, • Listens to others without interrupting and keeps emotions under control- Remains open to others' ideas and tries new things COMPETENCIES • Practices infection control and special precautions in all resident care duties, • Works with integrity; upholds organizational values, • Must be able to work semi-independently and have strong written and verbal communication skills, • Ability to read and interpret documents such as safety rules, operating procedures, policies and employee manuals, • Ability to write reports and correspondence, • Ability to speak effectively in front of others, • Ability to solve practical problems and deal with a variety of variables in situations, • High level of interpersonal skills to handle sensitive and confidential situations and documentation, and interaction with residents' families and families experiencing loss, • Attention to detail to ensure accuracy, • Must be proficient in English and able to read, write and follow instructions in English (Rule 111-8-63-.20 2b):, • Demonstrates and knowledgeable of the Six (6) Rights of Medication Pass ("Right" resident, medication, dosage, time, route, right to refuse) ORCHARD EXPECTATIONS: • Adheres to all Orchard policies and procedures, • Maintains absolute confidentiality of all information pertaining to residents, families, and staff and adhere to all HIPAA rules, • Displays a positive and professional image and attitude in all relationships with residents, families, peers and in the community- Outlined in detail in the employee handbook, • Maintains positive communication and team collaboration, • Serves as an ambassador for Orchard at all times, • Demonstrates ongoing responsibility for self-education, • Maintains resident and personal safety regarding occupational health and safety, • Attends regularly scheduled department meetings, • Responsible for maintaining all continuing education and training requirements for position, • Attends and participates in onsite training and continuing education as required, • Participates in safety program, as requested, • Cooperates to investigate employee injuries, • Promotes and support safety awareness and implement safety operations, • Participates in orientation of new employees, • Participates in volunteer orientations, training and education as requested, • Remains updated on all state regulations related to position, • Ensures compliance with all state and federal regulations (i.e. OSHA), • Participates in the risk management programs, • Demonstrates the ability to be innovative and creative and have patience, high energy and resourcefulness in problem solving, • Ability to work evenings, weekends and flexible hours when necessary to meet the needs of our residents, • Follows Professional Dress Code policies as outlined in the employee handbook for community and department specific standards, • Adheres to Orchard's No Cell Phone policy as outlined in the employee handbook at all times, • Perform other related duties as assigned.
