CAMBA’s Atlantic House Men’s Shelter located in the Brownsville section of Brooklyn serves 200 single men at any given time. The majority of these clients are dually diagnosed with mental illness and substance abuse. Position: Engagement Coordinator Reports To: Assistant Program Director Location: 2402 Atlantic Avenue, Brooklyn, NY 11233 What The Engagement Coordinator Does: The person filling this position is expected, under general direction, to ensure smooth day-to-day running and supervision of social services staff and services in accordance with program goals, targets, and performance outcomes and all CAMBA and program policies, procedures, and protocols, including the following: • Maintain professional relationships with clients and client confidentiality, • Practice Universal Precautions/Standard Protocol & Procedures, • Comply with any and all Federal, State, City and CAMBA security and privacy polices intended to protect the security and privacy of individual identifiable health information., • Communicate effectively with clients, direct reporting staff, peers, supervisors, and funders, • Provide direct interventions with the clients that are culturally appropriate and take into account the client's communication styles, values and life experiences, • Provide non-direct interventions that support the shelter team and increase communication between functions, • Conduct and document one-on-one supervision with one or more of the following staff; Job Description JOB SUMMARY: Under the general supervision of the Team Leader/Program Director, and the guidelines provided by the NYS OMH for the Mental Health Comprehensive Medicaid Case Management Program (MH CMCM), functions as part of a multi-disciplinary treatment team with shared caseloads, assists participating individuals who have serious and persistent mental illness in developing and maintaining viable living, working and social situations in the community by obtaining needed medical, social, psychosocial, educational, financial, vocational and other services. Has lead responsibility for integrating family goals and services with the tasks of all team members and for providing family psycho-education individually and in groups. Participates in the provision of crisis intervention services to participants 24 hours a day, seven days a week. Job Description THE ORGANIZATION The Center for Justice Innovation is a nonprofit organization dedicated to advancing community safety and racial justice. Since 1996, we’ve worked alongside communities, courts, and those most directly affected by the justice system to build stronger, healthier, and more equitable neighborhoods. With a team of over 900 staff and an annual budget of $130 million, the Center carries out its mission through three core strategies: • Operating Programs that pilot new ideas and address local challenges;, • Conducting original research to evaluate what works—and what doesn’t; and THE OPPORTUNITY Midtown Community Justice Center seeks a Community Navigator for the Community First program. The aim of Community First is to create an upstream diversion from arrest and justice system involvement by building supportive relationships with people in need of services and creating linkages to mental health services, housing support, substance use services, and other kinds of social services as needed. Community First is a partnership between Midtown Community Justice Center, Breaking Ground, Fountain House, Times Square Alliance, Housing Works, and Ryan Health. Reporting to the Clinical Director, the Community Navigator will be tasked with engaging community members frequenting the Times Square and Midtown Manhattan area who are experiencing housing insecurity or homelessness and often have co-occurring behavioral health challenges. In keeping with the Community First model, the Navigator will build trusting relationships with those individuals, and facilitate linkages to social services and other community-based resources. The Community Navigator will also work with clients of MCJC’s onsite programs, including court-based and diversion programs, who are unstably housed. Community Navigators are peers who share some lived experience with clients and use their own experience in establishing trusted relationships. if a client did not attend assist with rescheduling & addressing any barriers, • Utilize motivational interviewing if a client does not want to participate or be linked to treatment; attempt to understand your client’s perspective or misconceptions, remove any barriers, and attempt to re-engage, • Provide therapeutic groups (music, art, general support groups for caregivers or children); facilitate group work to address family issues, enhance life skills, etc., • Participate in family meetings with case management & housing staff to discuss planning efforts & progress, • Assist clients with completing applications for supportive housing and accompany residents to supportive housing interviews when necessary, • Upon linkage to permanent housing, complete and discuss exit summary with family, and provide the family with referrals and supportive services identified in new community, • Utilize fact sheets, desk guides, and other material resources in delivering mental health services, • Complete all documentation in a timely manner; The Soteria ICR will work with guests for up to 28 days to help them develop awareness and skills that will allow them to return to the community with an increased ability to manage their mental health concerns.
