Job Description Cayuga Counseling Services, Inc. is seeking a Full Time Wellness & Youth Services Coordinator. The Wellness & Youth Services Coordinator provides comprehensive support and coordination of services to individuals and families participating in Cayuga Counseling services programs. This position bridges health care navigation, case management, and youth and family engagement by assisting clients in accessing needed services, facilitating program activities, and fostering meaningful connections. The coordinator works under wavier and case management programs, while also planning and implementing youth-focused activities and group sessions that promote wellness, resilience, and life skills development. The position responsibilities include the following: • Provide Case Management and care coordination for clients under wavier and community-based programs, ensuring that services meet individual goals and support overall wellbeing., • Conduct intakes and ongoing assessments to determine strengths, needs and appropriate service plans., • Support clients and families in navigating healthcare, behavioral health, and community resources, advocating for access to care and continuity of services., • Facilitate and co-facilitate group programming, including educational workshops, support groups, and skills-based sessions for individuals, youth and families., • Coordinate and implement Kids Club and other youth activities, ensuring they are developmentally appropriate, inclusive, and aligned with program goals., • Plan weekly and monthly activities for Kids Club., • Maintain accurate detailed documentation in accordance with agency, program, and funding requirements., • Participate in community outreach and collaboration, building relationships with schools, healthcare providers and other community partners., • Contribute to program planning, evaluation and continuous improvement efforts., • Bachelor's degree or higher and at least 3 years' experience in the human services field. A combination of education and experience will be considered., • Must possess the ability to organize and prioritize duties to meet job demands, make independent decisions, and successfully engage with children and families., • Have a valid NYS driver's license & reliable transportation. Travel is required, • Proficient in Microsoft Office Suite, • have excellent verbal and written communication skills, • 3 Weeks PTO in your first year, 4 weeks in your second year., • 48 hours of wellness time annually (4 hours accrue per month) that can be used any time you feel you need some time to yourself., • 96 hours of sick time annually (accruing 8 hours per month), • 10 Holidays off per year including a Floating holiday., • Agency sponsored life and Dental Insurance for employees, family plans are also available., • 6 different health Insurance to choose from to fit your needs., • Sabbatical: After 5 years of service, you are eligible for 4 weeks paid time off that doesn't go against your other PTO Job Posted by ApplicantPro
