Job Description Fulltime Home Health Care visiting Physical Therapist needed our Cayuga County territory. Role and Responsibilities A qualified professional, who directs, supervises, evaluates and provides physical therapy services to patients in the home, following physician’s orders. Essential Functions • Evaluate patient using diagnostic muscle, nerve, joint, and functional ability tests and measures., • Prepare an assessment of the patient’s condition at the time of the initial evaluation and perform periodic re-evaluations, develop a written problem list and plan of care including types and frequencies of treatment., • Recommend modification of plan of care to physician, as needed., • Document clinical notes on the date of the visit into medical record and keep all records and summaries up-to-date., • Treat patients to address impairments and functional limitations, with a goal of reaching maximum functional independence., • Instruct and/or recommend patients and their caregivers in the use of prosthetic, orthotic, and durable medical equipment (canes, walkers, wheelchairs, etc.)., • Include and educate the patient and caregiver(s) regarding treatment plan and goals., • Attend physical therapy staff meetings and in-service presentations., • Participate in case conferences., • Participate in the development and periodic revision of the physician’s Plan of Treatment., • Assure appropriate safety practices and maintenance of equipment., • Other duties as assigned.
