Job Description & Requirements • Specialty: Physical Therapist, • Discipline: Therapy, • Start Date: ASAP, • Duration: 13 weeks, • 40 hours per week, • Shift: 8 hours, days, • The physical therapist will accurately assess, differentially diagnosis, and identify deficits in a patient’s ability to move and perform functional activities in their daily lives. They develop an individualized treatment plan with patient-centered goals to reduce pain, restore function, and prevent disability and work with the individual to prevent the loss of mobility.\n\t, • 2 years PT experience - Required\n\t, • erforms patient evaluations through the process of obtaining history, performing relevant systems review, and selecting and administering specific tests and measures to obtain data.\n\t, • \n\tEstablishes an individualized plan of care and selects treatment techniques that addresses the specific impairments identified in the evaluation and in accordance with physician orders.\n\t\n\t, • \n\tConducts ongoing reassessment of the patient and their response to treatment.\n\t\n\t, • \n\tInitiates and manages the discharge plan beginning at the first visit and continuing throughout treatment.\n\t\n\t, • \n\tSupervises and delegates to ancillary staff and students consistent with state laws and department policy.\n\t\n\t, • \n\tCompletes legible, accurate and clear documentation within the established timeframe identified in the department policy and procedure and in accordance with accreditation requirements and insurance regulations.\n\t\n\t, • \n\tCommunicates with the patient, family, caregivers, significant others, and members of the health care team to promote maximum benefit of care utilizing AIDET.\n\t\n\t, • \n\tAssesses educational needs of the patient, family, caregiver, or significant other and provides education to meet those needs.\n\t\n\t, • \n\tParticipates in professional growth and development through participation in educational programs, including in-service meetings, workshops, and by reading current literature concerning scope of practice.\n\t\n\t, • \n\tActively participates in quality improvement activities.\n\t\n\t, • \n\tProvides an environment conducive to safety for patients, visitors, and staff.
