Trestle Programs, Inc. operates programs in 8 of the 23 Judicial Districts throughout Colorado. Were looking for the right candidate to join our team in the 10th JD in Pueblo, CO. GENERAL PURPOSE: The CYDC Youth Service and Support Partner is responsible for empowering and supporting youth with high-risk behavioral health issues and criminalistic behaviors by providing individualized, strength-based support, education, coaching, information, and resource referrals in the context of a child and multidisciplinary team.They will possess the ability to articulate and model lessons learned from lived experiences. The Youth Service and Support Partner also provides support and advocacy to families to ensure that family voice is heard throughout criminal proceedings and the juvenile justice process. The Youth Service and Support Partner reports to the CYDC Program Supervisor and CYDC Director. Specific Responsibilities: Court Liaison (5%): • Attends all docket days and detention hearings of current or potential clients., • Offers informational support to youth and family regarding juvenile court processes. Youth Support (65%) • Assist clients in navigating the complex juvenile justice system in Pueblo County, • Provide peer support and assistance with system navigation to families involved in multiple systems in the City and County of Pueblo, • Actively engage in Youth Service and Support Plan meeting and development, • Provide a full range of information, support, encouragement, advocacy, and referrals to appropriate community resources, and related services to assist the clients in coping effectively with immediate and/or long-term situations., • Participate in prosocial activities with youth., • Provide transportation for youth to appointments, school, activities as needed., • Actively participate in advocate role with key partners such DHS, schools, and other community partners, • Active involvement in community outreach and networking, • Monitor and support youth and family progress through the Youth Service and Support Plan, • Assist parents with meeting basic needs of family such as healthcare, housing, food, etc., • Participate in multi-disciplinary staffing teams including PART/FEM/FST, • Attend Court and other meetings as needed with youth., • Assist youth and family with identification and connection to community-based resources., • Conduct home/community visits as necessary to follow up on progress and/or to make contact when other means of communication are not effective., • Assist parents with scheduling/attending necessary appointments., • Empower and encourage families to actively participate in their Youth Support Plans and Program to obtain self-sufficiency without system involvement., • Assists youth in meeting short-and long-term goals and in accomplishing them., • Ensures the delivery of social services, emergency, and non-emergency services, directly or through referrals. Case Manager Specific Responsibilities (30%): • Assist CYDC Program Supervisor with risk assessment services to youth who enter the detention center, utilizing Colorado Juvenile Risk Assessment (CJRA)., • Assist CYDC Program Supervisor with developing a Detention Hearing Report in preparation for the Detention Hearing., • Monitor youth at home and school under Supervision guidelines (if applicable), • Prepare court reports., • Communicate with outside agencies., • Turn in documentation in accordance with time deadlines., • Manage the cases of appropriately screened pre-adjudicated and referred youth released into the community on CYDC services., • Make referrals to service providers., • Connect and disconnect EHM equipment for youth (if needed), • Monitor the whereabouts of youth on supervision., • Ensure client electronic files are accurately maintained and up to date., • Monitors youths progress in the services being provided., • Report progress and concerns to the appropriate professionals to include the courts., • Maintain ongoing contact with all service providers to include scheduled site visits., • Collect required data and enter Colorado Trails according to required timelines., • Actively participate in weekly team meetings, and regular case staffings., • Assist in coverage for employees who may be absent from work., • Always conduct self in a professional manner., • Other duties as assigned by the CYDC Program Supervisor and CYDC Director., • Provide support to CYDC management and staff., • Complete required professional development (if applicable). Qualifications: Minimum Qualifications • This position requires a High School Diploma or GED with at least two years experience working with at-risk youth and families. Bachelors Degree preferred., • Must be able to communicate professionally and accurately with outside contacts and youth and their families., • Must have positive interpersonal skills. Required Competencies (knowledge, skills abilities) • Ability to work in a self-directed and self-motivated way., • Ability to interface with interrelated and collaborative partners., • Ability to utilize and interpret statistical data., • Familiar with the juvenile justice system in Colorado and the Childrens Code., • Familiar with education system with an emphasis on special education., • Excellent communication and interpersonal skills., • Excellent organizational skills., • Ability to manage several projects at the same time while meeting deadlines., • Ability to problem solve and to make appropriate decisions in short timeframes., • Ability to comply with established rules, statutes, policies, and procedures., • Ability to be fair, impartial, and respectful of others beliefs, values and culture., • Ability to develop rapport with professionals from various agencies., • Proficient in Microsoft Office programs and ability to learn new software programs., • Training in and knowledge of motivational interviewing techniques. Preferred Qualifications • Bachelors degree in Criminal Justice, Education, or closely related field, • Experiencing with Evidence Based Practices and Programs, • Experience with data driven decision making., • Experience working on and with a multidisciplinary team (MDT), • Experience creating data collection spreadsheets.
