The Primary Care Physician (PCP) in our organization demonstrates: • Accountability for outcomes: The PCP demonstrates accountability for outcomes, strong clinical care, and cost-effectiveness for each patient in their panel of up to 450 patients. They understand that they can strongly influence the patient’s outcomes by building a trusting relationship and helping them change behaviors., • Coaching for health: The PCP acts as a health coach, rather than just a consultant for sickness, by helping patients set short and long-term health goals, partners with the patient to work toward the goals, and frequently follows up on those goals on the path to improved health for their patients., • Simplifying for action: The PCP simplifies and prioritizes appropriately so that behavior change is more actionable, both for the patient in helping them achieve their goals, and when leading their care teams towards their performance goals. We are an outcomes-focused, value-based organization and for their panel of patients, the following metrics are regularly measured to help PCP’s become successful and reach partnership status: patient admissions/thousand, using between 18-21 appointment slots per day (each new patient count for 2 slots, follow-up patients 1 slot), CGCAHPS (patient experience), clinical gaps closures, and medical cost measures. Each PCP will have goals for these metrics and will be expected to work towards those targets with their center and market leadership as well as their care teams. Culture is very important in the medical centers and because PCP’s are leaders in our organization and centers, they are expected to help champion a positive culture of love, accountability, and passion along with center leadership. The PCP will be required to demonstrate the ability to function both independently and in collaboration with other health care professionals. The PCP will work closely with the applicable managers and medical directors to ensure compliance with guidelines along with participating in risk and quality management programs, clinical meetings and other meetings as required that promote patient health and company goals. The PCP will adhere to strict departmental goals/objectives, standards of performance, regulatory compliance, quality patient care compliance, and policies and procedures. THE JOB As an Emergency Credentialed Veterinary Technician, you're part of a team that thrives on the front lines of emergency veterinary care where every second counts. You'll handle a wide range of cases, from minor to complex emergencies, collaborating with our doctors and team in delivering gold standard, emotionally intelligent care. You'll jump into triage, treatments, diagnostics, anesthesia, monitoring, and critical care nursing, all while being a steady, compassionate presence for pet parents in their most stressful moments. This role is for someone who wants to do the work they were trained to do (and more), continue growing their skills, and help people and their pets when they need it most. WHAT YOU'LL DO • Treat every patient and person like they matter—because they do! You keep people and pets together and stay by their side through tests, treatments, and everything in between, • Jump into action, delivering nursing care, treatments, and emotional support in the most critical moments, from triage to discharge, • Bring the meds, the monitoring, and the mastery—from IVs and CRIs to blood transfusions and beyond, • Communicate like a teammate and think like a leader, helping shape plans and keep care flowing, • Be ready to perform CPR like the trained RECOVER rockstar you are (not only do we offer training, we have the highest number of RECOVER CPR professionals and instructors in the industry!), • All the techy stuff—from diagnostics and imaging to sample collection and placing tubes, • Anesthesia, even for our most fragile patients? We are seeking a dynamic physician who will be an addition to our palliative care team in caring for Kaiser Permanente Georgia (KP GA) members with potentially life limiting illnesses. There will be a mix of inpatient and outpatient palliative medicine, but the primary focus is on outpatient care, providing assessments, treatment, and recommendations. The physician will work closely with the team’s nurses, chaplains, social workers, clinical pharmacy specialist, and other team members. Outpatient visits with members will primarily occur in KP medical offices and virtually. Inpatient work occurs at the two core hospitals, Emory St. Joseph’s, and Emory Decatur Hospitals. Collaborating with the member’s other physicians, the physician in this position will treat and/or recommend care for members' medical problems and illnesses with emphasis on patients’ quality of life and functional ability. This will include counseling on advance directives, physical and psychosocial concerns, end of life care, and other topics as appropriate. Ideal candidates must be board certified in internal medicine or family medicine or other relevant certifications and board certified in palliative care medicine. Major Responsibilities/Essential Functions: Conducts office, telephone, video, and inpatient visits as appropriate. Assesses, treats, and/or makes care recommendations for members referred to Palliative Care. Addresses clinical issues and develops medical aspects of care plan (included but not limited to recommendations regarding observations, lab tests, medications, referrals, ordering tests and prescribing medications as appropriate). Working collaboratively with the other members of the Palliative Care Team, as well as the member’s PCP, Oncologist, Cardiologist, and others involved in member’s care, to optimize the member’s overall health and functional status. Serves as resource for other clinicians, nurse, clinical pharmacy specialist (e.g., authorizing medication changes as appropriate), social worker, and chaplain to enhance care for members with potentially life limiting illness. Participates in Palliative Care-related meetings, including relevant workgroups and department meetings, reviewing team goals, activities, and quality metrics, and recommending interventions as appropriate to improve overall care for KP GA members with potentially life limited illnesses. Participates in community presentations, KP GA presentations, and TSPMG continuing medical education activities regarding Palliative Care as appropriate, to enhance awareness and knowledge of conditions and issues important for those facing a potentially life limiting illness. Also participating in ongoing educational activities to increase staff awareness of palliative care services in other departments. Minimum Education and Experience Requirements: • One-year post-residency experience in caring for palliative care patients in an outpatient and/or facility setting., • Demonstrated ability to effectively communicate with patients with potentially life-limiting illness, their family members/caregivers, and other physician and non-physician health professionals., • Two years post-residency experience in providing palliative care services to patients in the outpatient and/or hospital/facility setting., • Experience working as part of an interdisciplinary team.
