We are seeking a Full time RN FIELD SUPERVISOR who will be based at our Yonkers location. As a field supervisor, you will further Bestcare’s goal for our patients to function safely, at their optimum level of independence. Job Responsibilities: · Conduct initial home visits to assess patient needs and develop the Aide Plan of Care · Communicate with a patient’s physician and other team members to ensure delivery of appropriate patient care · Supervise patient care provided by our Home Health and Personal Care Aides · Participate in orientation, teaching, and evaluations of care providers · Responsible for the review and maintenance of patient records, using telehealth modalities for early intervention of patient issues Job Qualifications: · Must be a valid Registered Nurse licensed in New York State, preferably with two (2) years of experience · Must have a valid driver’s license and transportation. · Previous home care experience is a plus · Bilingual Spanish and English language abilities is a plus Benefits: • FLEXIBLE SCHEDULE, • Strong team approach and support Our commitment to providing consistent, quality care to our patients is the motivation for our care team.
