Job Description POSITION SUMMARY: The Family Practitioner is responsible for providing comprehensive health care and preventive services. The Practitioner diagnoses and treats diseases and injuries of human internal organ systems, examines patient for symptoms of organic or congenital disorders and determines nature and extent of injury or disorder, referring to diagnostic images and test, and using medical instruments and equipment. They are also responsible for prescribing medication and recommends dietary and activity program, as indicated by diagnosis. RESPONSIBILITIES: 1.Evaluation, diagnosis and treatment for patients with acute illness. 2.Preventive care, consisting of periodic physical examination, immunizations, health education and preventive screening. 3.Treatment, counseling, and referral for rehabilitation for maintenance of patients with chronic conditions. 4.Coordination of specialty services for consultation and treatments. 5.Referral and follow-up for patients requiring in-patient care at St. John’s Episcopal Hospital/SSD, Queens Hospital Center, Jamaica Hospital or others as required. 6.Will be required to be on telephone “on-call” coverage, for patients. 7.The Family Practitioner will work cooperatively with other allied health providers including the nurses, medical team assistants, counselors, social workers, psychologist, psychiatrist, pharmacist, nutritionist and laboratory technologist in providing comprehensive team health care. 8.The Family Practitioner is expected to participate in Continuing Professional Education programs to provide 50 CME credits per year. 9.The Family Practitioner will be required to participate in other Health Center related tasks as directed by the Medical Director. These may include but not limited to Quality Assurance, Patient Education in the Community, Completion of Survey, etc. 10. QUALIFIED NUTRITIONIST: Staff are thoroughly trained and knowledgeable in the use of the NYSDOH NYWIC computer system, where WIC participant information is documented. QN staff must have completed the educational requirements and practical experiences required to earn a degree and/or credential in the field of nutrition. This individual possesses expertise that qualifies them to perform all nutrition-related duties at the WIC agency. DUTIES AND RESPONSIBILITIES: • Demonstrates a high level of understanding of WIC’s mission, policies and procedures and program goals and objectives and appropriately communicates this information to participants, health care providers and community organizations, • Demonstrates high level of knowledge of code of ethics for WIC professionals, • Demonstrates advanced knowledge of food and nutrition requirements and dietary recommendations for women, infants and children in accordance with NYSDOH guidelines, • Conducts a complete nutrition assessment and determines nutrition risk in accordance with NYSDOH guidelines virtually or in-person, • Develops and implements individual care plans (ICPs) and oversees high risk care, tracks progress and documents outcomes, • Demonstrates advanced skills in using appropriate measurement techniques to collect and interpret anthropometric and hematological data, • Coordinates nutrition education that is participant centered, including breastfeeding promotion and support, that is responsive to the identified needs and interest of each participant, • Provides appropriate participant centered nutrition education for WIC participants in both individual and group settings as needed. Offers online nutrition education options to WIC participants., • Conducts breastfeeding assessments, and provides participant centered counseling that helps a mother initiate breastfeeding, establish milk supply, exclusively breastfeed or continue breastfeeding, • Addresses common breastfeeding concerns and issues breast pumps for mothers who meet eligibility requirements, • Demonstrates comprehensive understanding and appreciation for how sociocultural issues (e.g., race, ethnicity, religion, group affiliation, socioeconomic status and world view) impact participant’s nutrition behaviors and practice, • Documents nutrition services provided to WIC participants, including health and social services referrals and follow-up referrals, • Ensures referrals for lead testing, hemoglobin/hematocrit and immunizations are provided by WIC participants, as appropriate, • Prescribes, tailors, authorizes, issues and reissues food packages as needed, • Reviews and approves special formula requests requiring medical documentation in consultation with WIC participant’s Health Care Provider in accordance with NYSDOH guidelines, • Provides orientation and guidance to WIC participants to set up WIC health accounts for Online Nutrition Education, • Adheres to all regulations of confidentiality and participant safety as required by federal HIPAA regulations, USDA, the NYSDOH and JPA FHC, • Reviews Rights and Responsibilities with WIC participants and ensures understanding., • Responds to customer service inquiries about the WIC program and its benefits over the phone or in-person in a courteous manner. Provides additional information on topics such as WIC benefits and the Farmer’s Market Nutrition Program., • Attends conferences, professional seminars, in-service trainings, completes NYSDOH/ JPA FHC mandatory trainings and yearly continuing education credits to improve knowledge as needed, • Participates in breastfeeding promotion and support activities as needed., • Participates in nutrition education activities to promote WIC enrollment as needed. Assist with compiling or developing educational materials to utilized during nutrition education sessions or group nutrition classes as needed., • Assists to develop and/or update the local agency Nutrition Education and Outreach plan as needed., • Utilizes language line when speaking with participants that need translation services., • Performs any other related duties as required. MINIMUM QUALIFICATIONS: • Bachelors, or Master’s in nutrition degree from an accredited college or university, • Ability to perform tasks in a timely manner and follow instructions., • Ability to work flexible schedules, accommodating for rotation to different WIC sites based on program needs., • Demonstrates strong organizational, interpersonal, oral, and written communication skills., • Strong computer skills with knowledge of Microsoft Windows, experience with data entry preferred., • Bi-lingual (English/Spanish) preferred.

This position oversees a team of Nutritionists and a Breastfeeding Peer Counselor, ensuring the delivery of high-quality services through staff training, supervision, performance evaluations, and program development. The Nutrition and Breastfeeding Coordinator also plays a key role in policy implementation, quality assurance, event planning, and community outreach. RESPONSIBILITIES: Core Program Knowledge and Professional Standards • Demonstrates a high level of understanding and commitment to WIC’s mission, program goals, policies, and procedures., • Adheres to all regulations of confidentiality and participant safety as required by federal HIPAA regulations, USDA, the NYSDOH and JPA FHC., • Maintains up-to-date knowledge of WIC program requirements, benefits and eligibility criteria., • Attends conferences, professional seminars, in-service trainings, completes NYSDOH/ JPA FHC mandatory trainings and yearly continuing education credit requirements as assigned. Leadership and Program Support • Assists in standardizing program operations across different WIC sites, acting as a site manager to maintain consistency and program integrity., • Assists in planning, implementing and evaluating WIC services to ensure compliance with NYS Department of Health WIC policies and guidelines., • Represents the WIC program at internal and external meetings as needed., • Serves as a backup Breastfeeding Expert as a Certified Lactation Counselor (CLC) and Qualified Nutritionist (QN) as needed and helps manage site operations across multiple locations., • Manages and maintains the breast pump inventory in accordance with state guidelines, staying informed on current models and participant preferences., • Maintains and ensures the cleanliness, accessibility and welcoming environment of breastfeeding rooms at each WIC site. Supervision, Training and Staff Development • Directly supervises and evaluates WIC Qualified Nutritionist and Breastfeeding Peer Counselor, providing guidance, and performance feedback. Conducts annual performance reviews and state mandated quality assurance surveys., • Coordinates daily operations and workflow to ensure efficient client services and administrative processes are followed consistently across sites. Identifies and addresses staffing needs, workflow and scheduling deficiencies with WIC Director and Assistant Director., • Develops, implements and monitors orientation and ongoing training programs for new and existing staff in accordance with state and local agency policies., • Maintains training documentation and ensures staff meet required competencies and certifications for both state and JPA FHC mandatory trainings with assistance from Administrative Assistant. Communication, Meetings and Annual Events • Assists with WIC Program’s SharePoint platform to support interdepartmental organization and access to primary documents., • Disseminate relevant program updates, training opportunities and quality improvement initiatives to staff., • Assists with monthly nutritionists’ staff meetings to provide updates, reinforce policies, share best practices and foster team engagement., • Plans and manages WIC sponsored events in collaboration with WIC staff including logistics, promotion, and evaluation including but not limited to National Nutrition Month, Breastfeeding Awareness Month and Annual WIC Baby Shower. Quality Assurance & Program Compliance & Initiatives • Assists in monitoring program data and quality indicators to ensure compliance and efficiency., • Supports with preparation for program audits, state site visits, and other quality assurance activities., • Leads strategic initiatives to increase online nutrition education frequency of WIC program., • Promotes breastfeeding initiatives across sites for improved maternal and infant outcomes., • Collaborates with the Outreach Coordinator and community partners to expand breastfeeding and nutrition awareness in the community., • Performs any other related duties as required. MINIMUM QUALIFICATIONS: • Bachelor’s degree in nutrition. Masters Preferred., • Maintains active certification as a Certified Lactation Counselor (CLC), • At least three (2) years of experience with WIC or a similar food assistance program., • Supervisory experience in a clinical, community health, or administrative setting., • Strong knowledge of maternal and child nutrition and lactation support., • Ability to lead and motivate a team in a fast-paced environment., • Excellent organizational and time management skills resulting in the ability to manage multiple projects and prioritize competing deadlines., • Strong interpersonal and problem-solving skills., • Demonstrates excellent presentation and public speaking skills., • Knowledge of Microsoft Office, experience with data entry preferred., • Familiarity with local community resources and populations served by WIC., • Demonstrates cultural competence and sensitivity in working with diverse populations., • Ability to work flexible schedules, accommodating rotation to different WIC sites based on program needs.
