WHO IS OUR IDEAL RESIDENT CARE COORDINATOR We are looking for an experienced, energetic, and compassionate Resident Care Coordinator who can manage and support our direct care staff. This position has a lot of opportunity for growth and has a direct path to an administrator position(s) within our Company. As the Resident Care Coordinator, you will lead a team of caregivers and med-tech staff alongside the community Registered Nurse and Facility Administrator. JOB DUTIES AND RESPONSIBILITIES OF A RESIDENT CARE COORDINATOR The responsibilities of the Resident Care Coordinator include, but are not limited to: • Develops, executes and maintains resident care plans that meets the individual needs of the resident and the physician recommendations., • Work with the nurses, med-aide and caregivers to build relationships and strong communication with residents, families and associates., • Maintain a documented system of operations policies and procedures and ensure that all facility practices and policies are carried out in the highest ethical manner. Make recommendations for new or changes in procedures, policies, methods, education, reference material and general resident care and nursing practices to ensure the highest level of quality patient care is given., • Participates in the manager on duty and clinical on-call rotation as assigned; including weekend on-call duties and covering clinical shifts as necessary to ensure resident care needs are met., • Assist Management with the human resource function, including the design of an organizational structure adequate for achieving the Company's goals and objectives, recruitment, training, performance evaluation of staff., • Oversees the following day-to-day operational functions:, • Oversee all direct care staff (caregivers, med-techs, cooks) and ensure they are performing duties as required by Resident Service Plans, • Handle staff scheduling and ensure all shifts are adequately covered., • Overseeing medication administration, ensuring adequate ordering, receiving, documentation, and safe storage of physician-prescribed medications for residents. Conducts regular review of medication documentation., • Make daily rounds to ensure that all direct care staff are performing their work assignments., • Coordinates the scheduling of resident appointments and services for regular and emergency medical, dental, foot care, eye examinations and auditory testing ensuring follow up actions are documented in resident records and communicated to staff. Maintains contact with hospital when resident is transferred. Serves as liaison between residents and physicians, home health care agencies, pharmacies and ancillary services., • Collaborates with nutritionist/cook/kitchen staff to guarantee that the nutritional needs of residents are met at each meal according to Resident Service Plans. Provides quality assurance of all aspects of the resident dining experience., • Plans activities of residents as required by State guidelines This position is a full-time salaried position, with workdays from Sunday through Thursday. Normal business hours are from 7:00 AM to 4:00 PM with the understanding that hours may vary dependent upon workload, management and resident needs. Must be able to be on-call, including during inclement weather, evenings, weekends, and holidays. If LPN, Must possess a current, unencumbered, active license to practice as an LPN in this state;, • Must have at least Two (2) or more years of experience in a supervisory/managerial capacity with progressive levels of responsibility;, • Knowledgeable of nursing and medical practices and procedures, as well as laws, regulations and guidelines that pertain to residential care facilities;, • Experience with Electronic Medical Records and computer documentation systems;, • Experience and knowledge of current federal and state healthcare/assisted living/residential care facility rules and practices;, • Working knowledge of Microsoft 365 office technology and software (MS Sharepoint, MS Teams, MS Office apps: Office: Word, Excel, Outlook, etc.), • Excellent writing and communication skills to work effectively with subordinates, residents, managers, and others;, • Be able to lift up to 30 pounds, aid in resident transfers, walk/stand 80% or more of shifts, and respond to all resident and emergency needs quickly, with or without reasonable accommodation., • All new staff hired must provide acceptable evidence of vaccination or become vaccinated for COVID-19 immediately upon hire as a condition of employment.
