Essential Functions of the Role: The Intensive Case Managers operate within a multidisciplinary unit and include Client Care Coordinators. All Intensive Case Managers have at least 2 years clinical experience, which includes client direct contact experience. function as an advocate, facilitator, outreach coach, educator, care coordinator, and motivational counselor for members and their families for members who have complex behavioral health and or medical conditions. The role of the Intensive Care Manager includes, but is not limited to the following tasks: Position Responsibilities: • Providing a timely outreach to new referrals, • Engaging members into the program by providing compelling rationale on the benefits of the program to fit the unique member's needs., • Completing members needs assessment to determine appropriate services and inform the care plan., • Developing an individualized member centric comprehensive care plan with input from the member, provider, and family. The individual goals include recovery and resiliency, decreasing symptomatology and/or increasing functional ability in areas such as self-care, work/school, and family/interpersonal relations to reduce barriers to treatment., • Providing monitoring and reviewing of cases through planned outreach, incoming contacts, care coordination and utilizing rounds, weekly reports, and individual supervision., • Rounding or staffing with a supervisor takes place once per month at a minimum for difficult or challenging cases., • Providing consultation and coordination with the behavioral health or medical providers, facility or family members, community agencies, or involved medical practitioners regarding treatment and/or treatment planning issues., • Providing motivational counseling and encourage self-advocacy to help sustain members' commitment to their care plans and treatment adherence., • Coordinating and consulting with the Care Manager as necessary. Attending regularly scheduled rounds to consult with a psychiatrist or health plan staff and discuss cases and the need for continued intensive care management and outreach. Sending outreach letters to members who are not telephonically accessible or who do not res pond to multiple telephonic outreach attempts., • Frequency of outreach to the member, supports and provider(s) occur at a minimum one time per month, but more may be scheduled according to the member's clinical needs., • Send outreach letters to members who are not telephonically accessible or who do not respond to multiple outreach attempts., • Client's progress and Intensive Case Manager interventions are documented appropriately in the care management system., • Provide case closure/discharge at the time of completion., • Follow all workflows meeting regulatory and accreditation requirements., • Education: 1. A bachelor’s degree in one of the fields listed below; or 2. A NYS teacher’s certificate for which a bachelor’s degree is required; or 3. NYS licensure and registration as a Registered Nurse and a bachelor’s degree; or 4. A Bachelor’s level education or higher in any field with five years of experience working directly with persons with behavioral health diagnoses; or 5. A Credentialed Alcoholism and Substance Abuse Counselor (CASAC). Salary Range: $42,500 - $47,000

Address immediate and emerging needs, set goals, resolve issues, advocate and connect clients to the needed resources such that their lives become stable, recovery centric and interactive. Enhance coordination of medical and behavioral health care with the main focus on the needs of persons with multiple chronic illness. Essential Functions of the Role: • Assess documentation for compliance as well as assessing staff performance and accountability, • Assess notes for quality and attentiveness, determine whether there is full engagement and determine whether assistance is needed with direction and resources, • Assist as needed with referrals to community linkages and social support services such as obtaining benefits and entitlements, food, shelter, permanent housing, • Assist individuals to help connect to natural supports that enhance the quality and security of life, • Assist Staff with developing crisis diversion plans or relapse prevention plans, • Assist/coach peer support workers in how to best engage in both self-care and client care in an effective way with a goal of maintaining recovery and stability, • Attend all trainings and meetings as assigned, • Conduct monthly meetings per OMH and County guidelines, • Create a venue for peer support staff to talk about their experiences in working with our population and whether/how it impacts their MH and recovery process, • Increase staff awareness of existing services, pathways to recovery and helping a person to remove barriers that exist for access to them, • Meet with team members to provide case direction and strategies, • One to one peer support using person centered goal planning that incorporates life areas such as community connectedness, physical wellness, spirituality, employment, self-help, • Outreach and Engagement: Companionship and modeling of recovery lifestyle, including participation in recovery activities that might be beyond the scope of treatment providers (e.g., eating together at a restaurant, attending or participating in a sporting event, attending a social event such as a concert or recovery celebration event), • Providing peer support in the person’s home or in the community to support them before (or in) a crisis or relapse, • Recovery Supports: Recovery education and coaching for individuals and their family members., • Remain passionate about quality client care, relationships and interactions with staff, • Use lived experience to draw empathy and to model recovery while ensuring the interactions are focused on client needs and client focused., • Bilingual English/Spanish a plus., • Computer literacy required., • Minimum 3 years’ experience, • NYCPS (New York Certified Peer Specialist). Permanent Certification preferred., • Tenacity and passion for this work with the ability to balance objectivity with empathy., • The role will be filled by a peer (a person with lived experience), • Valid NYS Driver’s License required and a driving record that will permit the use of an Agency vehicle to transport clients to appointments or other activities as needed. Job DescriptionSalary: $24 to $36/hour SPCA Westchester is seeking part-time and full-time Veterinary Technicians to join our talented staff of medical professionals at Codys Clinic. The ideal candidate will be an experienced vet tech who enjoys working with a mission-driven organization and a caring and energetic team. Veterinary Technicians are responsible for the care of clinic animals and assisting the Veterinarian in their duties. Job Classification: Part-time (schedule based on availability) and Full-Time (four 10-hour days) positions available.
