In this role, you will provide comprehensive occupational therapy services to senior participants enrolled in our community-based care programs, helping them remain safe, independent, and supported at home. Therapists work across multiple care settings including day centers, hospitals, skilled nursing facilities, assisted living facilities, and participants’ homes, and collaborate as part of an Interdisciplinary Team (IDT). This position is ideal for clinicians who enjoy autonomy, meaningful relationships, and a mix of on-site and home-based care. Travel to multiple program sites may be required. What Are the Responsibilities? • Complete initial, semi-annual, annual, and unscheduled assessments, • Participate in daily IDT updates and report changes in participant status, • Conduct comprehensive evaluations across ADLs, functional status, cognitive function, mobility, vision/perception, feeding, safety, home environment, and DME needs, • Develop individualized care plans and deliver treatment based on participant goals, • Provide therapy across all settings including participant homes, hospitals, and facilities, • Train and supervise restorative aides and caregivers on therapy programs and safe body mechanics, • Lead education for staff, participants, and families on occupational therapy services, • Make recommendations for DME, submit orders, and follow up on equipment delivery/service, • Assist with Quality Improvement (QI) initiatives, • Complete required documentation and reports in a timely, accurate manner, • Participate in meetings, in-service trainings, committees, and department initiatives, • Maintain flexibility in scheduling to meet participant needs, • Support organizational policies and maintain confidentiality at all times What Are the Benefits? • Medical, dental, and vision insurance, • 401(k) and retirement plan, • Paid sick time and paid time off, • Employee assistance program, • Health savings and flexible spending accounts, • Life insurance, • Employee discounts, • Referral program Where?
