RESPONSIBILITIES/ACCOUNTABILITIES: Implements treatment plans and programs through consistent daily and weekly schedules that focus on functional and measurable outcomes to be shared with the school and/or supervisors. Delivers specialized individual and group physical therapy instruction as mandated in students’ Individualized Education Plans (IEPs). Collects data and completes reports regarding the student’s goals and progress. Consults with other professionals (Teachers, SLP, OT, PCP, etc) on the student's treatment team to ensure a cohesive and comprehensive therapy team. Assesses and documents each student’s progress through formal and informal tests and measurements. Writes and prepares annual review reports and goals for each student in accordance with their IEP dates and requirements in order to remain in compliance with state guidelines. Provides physical therapy expertise when developing FBA/BIP. Provides parent and caregiver support and education to discuss the student’s progress and strategies for continued support outside of school. Attends all school related meetings as scheduled, such as, but not limited to, Triennials, Annual Reviews, Domain, Referrals, and pre-IEP meetings for each individual student. Ensures a suitable learning environment by setting up and preparing materials and supplies as needed. Supervises PTA(s) in accordance with state practice act guidelines, as needed. Communicates and follows up with schools and families to obtain appropriate physician signatures/orders for school-based physical therapy referrals in order to initiate and continue with services. Follows the safety guidelines and directives for the student in regards to functional mobility and orientation (i.e. transfers, wheelchair or walker mobility, etc.) and exercises provided by other related service providers (i.e. occupational therapist, vision itinerant, hearing itinerant, speech and language pathologist, orientation and mobility instructor, etc.). Utilizes communication devices and assistive technology as outlined by the speech language pathologist and itinerants. Follows any fire drill or safety guidelines outlined for each individual student. Provides screenings and evaluations to determine requirements for skilled physical therapy intervention relative to student/client’s level of function. This includes, but is not limited to the following areas: school function, range of motion, strength, positioning, contracture management, conducting home assessments, discharge planning, development of maintenance programs and other related client care issues. Provides documentation to maintain the quality of therapy records, as it relates to reimbursement guidelines, accuracy of billing information, and school requirements. Directly participate in the delivery and documentation of these services, when necessary., • Review all Restorative Services weekly to ensure all appropriate services have been delivered and submit Medicaid Billing Reports on monthly basis as per program protocols., • Oversee and participate in all Medicaid billable activities and ensure that all documentation is current and in compliance with regulatory guidelines. This includes timely and accurate submission of monthly Medicaid Billing Reports to the Billing Dpt., • Oversee all medication monitoring procedures and assume any medication management related tasks and responsibilities in the absence of a Senior Counselor. Housing Supervisors are to review the medication logs/EMAR on a daily basis to ensure all procedures are being followed appropriately., • Oversight of the sites to ensure a clean, safe and homelike environment. Ensure proper maintenance and upkeep of the site in collaboration with the facilities and management team., • Ensure all residents are treated with dignity and respect and are given the opportunity to actively participate in their service and support plans in order to provide collaborative treatment panning., • Organize and encourage respectful interactions between residents, conduct house meetings and offer supports and training as needed to residents in the program., • Conduct intakes for potential residents in cooperation with the admissions committee., • Review and approve all time off requests, time sheets and travel expenses for staff within 48 hours of submission., • Oversee program finances, including weekly client distributions, petty cash, fee payments, check requests, food and supply budgeting and ensuring appropriate funding and benefits are in place for all residents., • Oversight of recreational activities., • Serve as a liaison with outside service providers and advocate for the needs of residents in the program, when necessary., • Ensures that appropriate discharge planning activities occur for all residents., • Responsible for routine oversight and monitoring of all client records to ensure that records are up to date and include all required documentation in compliance with applicable individual program regulations and policy manuals., • Provide direction and support to staff during crisis situations. Serve as the On-Call Supervisor one week (wed-wed) after business hours and weekends on a rotating basis (approximately every 6-8 weeks) to answer any calls, text or Microsoft Teams messages in which staff and/or clients may need to report pertinent information, need assistance, handle a crisis situation or incident/accident in program within the Residential Dpt.
