Job DescriptionDescription: Primary Care HIV Prevention (PCHP) is a U.S. Health Resources and Services Administration funded program aimed at decreasing the risk of HIV transmission, focusing on supporting access to and use of pre-exposure prophylaxis (PrEP). The PCHP program focuses on increasing diagnosis, treatment, and prevention of HIV at JPAFHC and in the communities served by JPAFHC. The PCHP Prevention Educator will work closely with current JPAFHC patients and staff as well as the community to provide HIV education and health promotion services, outreach, HIV testing on- and off-site, and linkages to care. This role will play an integral part on the team to counsel, test, and refer at-risk communities to vital HIV, sexual health, and other primary care services at JPAFHC. They will also work closely with the Outreach Coordinator to provide testing, education, and referrals at outreach and partner events in the community. Requirements: 1. Engage JPA patients and community in health education workshops, HIV screening, counseling, and sexual health promotions, 2. They are responsible for the oversight and implementation of the Teen Advocate Project (TAP). This position facilitates evidence-based curricula and outreach to youth and adults in Nassau County. The Advisor/Sexuality Educator develops programming for the participating youth who engage their peers with sexual and reproductive health information and care they need using a variety of approaches, including workshops, outreach, community engagement, events and social media. The Advisor/Sexuality Educator will plan and implement the daily operations of the youth development program, manage the youth focused social media platforms, increase meaningful outreach to youth in the priority areas. The Advisor/Sexuality Educator will inform, educate, and raise awareness for SRH and SRJ health related topics to engage youth in Hempstead priority areas. The Advisor/Sexuality Educator will promote PPGNY services to youth in Hempstead through the Teen Clinic program. Outposted Therapeutic Housing Units Program (OTxHU) Since 2016, Correctional Health Services (CHS) has been the direct provider of health care in the New York City jails. Deeply committed to human dignity and patient rights, CHS is part of the NYC Health + Hospitals system and is a key partner in the City’s efforts to reform the criminal-legal system. Our in-jail clinical services include medical, nursing, and mental health care; pharmacy services; substance-use treatment; social work; dental and vision care; discharge planning; and reentry support. Given the high visibility of this initiative, we are seeking the highest caliber health care professionals in key clinical services to staff our Outposted Therapeutic Housing Units (OTxHU). To be located in three NYC Health + Hospital acute care facilities, the OTxHU is a pioneering approach to safely increasing access to high quality clinical care for patients in custody who have complicated health conditions. OTxHUs will bridge the gap in the continuum between care provided in the jails and inpatient hospitalization, with admission to and discharge from the OTxHU in accordance with a patient’s clinical needs.
