The Medical Social Worker participates in the interdisciplinary care provided to home health patients. The Medical Social Worker functions to evaluate and develop a plan of care personalized to fit the patient’s emotional and social needs. The Medical Social Worker provides direction and supervision of the Social Worker Assistant as required and when involved in the patient’s plan of care. The Medical Social Worker works within CenterWell Home Health’s company-specific policy and procedures, applicable healthcare standards, governmental laws, and regulations. * Assesses the patient’s social and emotional state as it relates to his or her illness or injury, needs for care and his or her response to such treatment, and adjustments to care. * Assesses any relationships of the patient’s medical and nursing needs in the home setting, financial resources, and available community resources. * Provides any appropriate action to obtain available community resources to assist in resolving issues that may be impeding the patient’s recovery. * Instructs patients and families in treating and coping with social and emotional response connected with Provides ongoing assessment of patient and family needs and responses to teaching * Assists the physician and other health team members in understanding the significant social and emotional factors related to the patient’s health Participates in the development and periodic re-evaluation of the physician's Plan of Care for the patient. * Observes, records, and reports changes in patients’ condition and response to treatment to the Clinical Manager and the Participates in the discharge planning process * Participates as a member of the interdisciplinary care team in care coordination activities and acts as a resource to other health team members in the identification and resolution of patient needs * Supervises instructs and evaluates the performance of the Social Work Assistant (BSW) to assure that all medical social services are provided to patients in compliance with Company, government, and professional standards * Maintains and submits documentation as required by the company and/ or facility including any case conferences, patient/physician community contacts, visit reports progress notes, and confers with other health care disciplines in providing optimum patient. Required Skills/Experience * Masters or doctoral degree from a school of social work accredited by the Council on Social Work Education. * Social Worker licensure in the state of practice; if required by state law or regulation. * A valid driver’s license, auto insurance, and reliable transportation are required. * Proof of current CPR certification * Minimum of one year of experience as a social worker in a health care setting, home health, and/or hospice. * Knowledge of and the ability to assist with discharge planning needs, and to obtain community resources (housing, shelter, funeral/memorial service arrangements, legal, information and referral, state/federal financial and medication programs, and eligibility. * Excellent oral and written communication and interpersonal skills. * Must read, write and speak fluent English. * Knowledge of medications and their correct administration. * Ability to organize tasks, develop action plans, set priorities, and function under stressful situations. * Ability to be flexible in work hours and travel locally. * Ability to communicate effectively with patients and their family members and at all levels of the organization. * Maintains current licensure certifications and meets mandatory continuing education requirements. * Must read, write and speak fluent English. * Must have good and regular attendance. * Performs other related duties as assigned. * Valid driver’s license, auto insurance and reliable transportation. Inspect, remove, and report any damaged racks or clips. * Rack and unrack parts while verifying correct part numbers and chemical processes as per work orders. Ensure no overloading or air pocket formation occurs during handling. * Operate material handling systems, including cranes, hoists, and carts, safely to transport parts and materials through processing areas. * Operate and monitor cleaning, Dow Coating, anodizing, dyeing, alodine, and rinsing tanks. Determine correct dip times and maintain appropriate temperatures and solution levels. * Perform specific treatments such as sulfuric/chromic acid dips on 75 ST parts formed on Cerro Bend. Apply alodine touch-ups or chromadize as required. * Assist in maintaining chemical strength by accurately measuring and adding solutions. Estimate material surface area to prevent generator overloads. * Start generators, operate control panels, and utilize overhead cranes to load/unload tanks and transfer processed items between stations. * Strip anodizing from racks using caustic soda and nitric acid. Clean tanks, crocks, and equipment to ensure safe and efficient operations. * Mark, oil, wrap, and route completed parts according to drawings or work orders. Ensure correct parts are matched to work orders and transferred to Quality Assurance (QA) for inspection. * Handle and dispose of hazardous materials (e.g., alodine, acids, solvents) in accordance with safety procedures. Complete all required on-the-job training and maintain necessary certifications. Overview: The Sr. Project Engineer role will oversee and execute the project controls and management of a multiple discipline, EPC project. Description: CARDIAC DEVICE NURSE-PERFORMS INTERROGATION, EVALUATION, AND REPROGRAMMING OF ALL MODELS OF CARDIAC RHYTHM MANAGEMENT DEVICES MUST BE ACLS-EXPERIENCE IN TRIAGING REMOTE TRANSMISSIONS AND PROGRAMING RECOMMENDATIONS - Job Summary: Provide nursing care to a defined group of patients, adhering to Kaiser Foundation Health Plan of Washington ambulatory Nursing Care Standards, the Nursing Code of Ethics and Kaiser Foundation Health Plan of Washington policies.Coordinate and facilitate Health Care services for the patient in other segments of the Health Care delivery system. Provide and coordinate nursing care for the patient in collaboration with the Health Care team. The Registered Nurse is an integral member of the Health Care team, overseeing and participating in patient assessment and the formulation and implementation of the care plan. Essential Responsibilities: • Using the nursing process, provides ongoing assessment of the nursing care needs of a defined group of patients. Uses nursing diagnosis to describe this assessment. Develops a written plan of care for patient care needs. Assists patients and families/significant others in developing health care goals which are part of the individualized plan of care. Determines priorities of patient care based on essential patient needs and available resources. Provides nursing interventions safely and competently. Provides the patient and family/significant others with information preceding interventions. Provides telephone consultation to patients and family/significant others Provides ongoing evaluation of the patients response to care. Anticipates and recognizes changes in the patients status and need for nursing care. Modifies the individual plan of care in collaboration with the health care team., • Demonstrates understanding of the physiologic and psychological effects of normal aging by assessing, planning, implementing and evaluating care to meet the needs of those populations served. Demonstrates effective communication skills with those specific age populations served. Identifies own learning needs and participates in educational programs related to those age-specific patient populations served (Neonatal, Pediatric, Adolescent, Geriatric). Collaborates, communicates and coordinates with other health care team members in planning and implementing care. Assesses the needs of designated patients requiring hospitalization; initiates nursing admission database and discharge planning in collaboration with inpatient staff and physician. Cooperates and communicates effectively with other team members in order to accomplish the workload., • Communicates recognition of the patients and family/significant others psychosocial, cultural and emotional needs in each nurse-patient interaction. Orients patients and family/significant others to the role of the professional nurse and other health care team members, the nursing services available, and the means of access to those services. Reinforces orientation to the Consumer Bill of Rights and Responsibilities. Identifies the patients educational needs as part of the nursing assessment. Develops an educational component for each nursing care plan. Provides teaching/counseling congruent with patients and family/significant others learning needs and the medical plan of care. Encourages patient compliance with therapeutic and medical regimens. Teaching is based on scientific and behavioral knowledge., • Assesses patient lifestyle risk factors. Integrates health promotion and preventive aspects of care with the ongoing assessment of nursing care needs. Works with patient to develop a plan to improve general health status. Implements physical and psychological preventive measures. Promotes patient self-care responsibility for meeting their own health needs. Adheres to KFHPW documentation standards. Provides a safe environment for each patient and adheres to KFHPW safety standards. Adheres to KFHPW infection control standards and guidelines, including the guidelines for reporting communicable diseases., • Identifies own learning needs and participates in continued education programs to meet those needs. Participates in staff orientation and student education activities. Collaborates with managers in completing the process for self and colleague/team performance reviews. Assures quality of care through development, implementation and evaluation of professional nursing standards and participation in multidisciplinary quality assurance activities. Participates in professional activities. Participates in nursing research and incorporates research findings into nursing practice. Adheres to KFHPW personnel policies and maintains a high standard of professional work habits.
