Job DescriptionBenefits: • 401(k), • 401(k) matching, • Health insurance The Program Manager provides leadership and supervision for program staff, including the Community Health Worker (CHW) Coordinator and Community Health Workers (CHWs), and ensures high-quality service delivery through a team-based, trauma-informed, and community-centered approach. The Program Manager also serves as a key liaison to subcontracted service providers and community partners and represents the organization in a professional capacity throughout the Network region. This is a mid-level management position with responsibility for both strategic program oversight and hands-on operational leadership. Key Responsibilities Program Administration & Oversight • Manage the development, implementation, and monitoring of annual program work plans in collaboration with staff, subcontractors, and community partners., • Ensure timely, accurate completion and submission of all required monthly, quarterly, annual, and ad hoc reports., • Monitor program performance, service delivery, and outcomes; conduct annual reviews and recommend program modifications based on data, community feedback, and identified needs., • Serve as liaison to subcontracted service providers, including negotiating essential agreements, monitoring progress toward targets and milestones, and addressing performance or compliance issues., • Facilitate meetings with subcontractors, community agencies, and county departments to provide program updates and assess progress., • Assist with research activities and support the preparation and development of grant applications, as needed.Supervision & Team Leadership, • Provide direct supervision and support to the CHW Coordinator and assigned program staff., • Ensure effective supervision of CHWs, including oversight of case management practices, documentation, and service planning., • Identify staff training and professional development needs and ensure access to appropriate perinatal health, outreach, and service delivery training., • Promote a collaborative, accountable, and supportive team culture.Community Health Worker Program Oversight, • Oversee CHW service delivery for short- and long-term clients, including high-risk clients and situations., • Ensure regular case review processes are in place, including review of client reports, service plans, and aggregate caseload data., • Review monthly caseload analyses and trends with supervisory staff to inform program planning and quality improvement., • Support outreach strategies to reach women of reproductive age in communities identified as high risk for poor birth outcomes.Community Engagement & Education, • Conduct community outreach to promote the organizations mission, perinatal health programs, and supportive services., • Attend meetings and events across the Networks service area to support county-specific initiatives and represent agency programs., • Conduct or coordinate community and professional education sessions related to prenatal, preconception, inter-conception, and perinatal health., • Conduct focus groups, consumer surveys, and provider surveys to assess maternal and child health needs., • Coordinate program-specific initiatives, such as smoking cessation activities, as assigned., • Provide client education and referrals through the organizations information phone line and Network programs, as needed.Areas of Expertise, • Knowledge of maternal and child health issues and the organization and delivery of health and human services., • Experience with community-based outreach, education, and case management models., • Familiarity with public health insurance programs and community supportive services., • Experience supervising staff engaged in outreach, education, and client support., • Strong written and verbal communication skills., • Proficiency with computer systems, databases, and reporting tools.Minimum Qualifications, • Bachelors degree in human services, nursing, social work, public health, or a related field required; clinical licensure (RN/PHN, LCSW) preferred., • Minimum of two years of supervisory and program management experience., • Experience working with high-risk populations and community-based service delivery., • Ability to work independently and collaboratively as part of a team., • Ability to work flexible hours, including some evenings., • Reliable personal vehicle, valid drivers license, and insurance; no travel restrictions., • Demonstrated dependability and professionalism.Preferred Qualifications A Public Health Nurse (PHN) or Registered Nurse with a Bachelors Degree in Nursing (BSN) or a Licensed Clinical Social Worker (LCSW) with experience in public health. Experience with community organization, and clinical case management (focused on individual and family).
