Job Description Nurse The Dutchess Reintegration Forensic ACT team (Assertive Community Treatment team) is a multidisciplinary team including clinicians, nurses, prescriber(s), and case managers. The FACT team is working at the forefront of the intersection between behavioral health and involvement in the criminal justice system. Our mission is to support our participants through innovative services that improve behavioral health, promote recovery and rehabilitation, create community success, and reduce incarceration. Services are delivered in the person’s home or community, available 24/7, and focus on helping individuals stay stable, avoid hospitalization, and build skills for independent living. The FACT Team Nurse is an essential member of the multidisciplinary Assertive Community Treatment (ACT) team, responsible for providing nursing care, health education, medication management support, and clinical coordination to individuals with severe and persistent mental illness. The nurse delivers services primarily in community settings—clients’ homes, shelters, and other natural environments—to support stability, recovery, and wellness. Essential Duties & Responsibilities: • Conduct health assessments, including physical health screenings, vitals, and monitoring of chronic conditions., • Administer medications, including long acting injectables, and ensure safe medication practices in the community., • Provide medication education to clients and families, including purpose, side effects, and adherence strategies., • Collaborate with the psychiatric provider to monitor medication efficacy, side effects, and overall clinical status., • Participate in daily team meetings to review client progress, identify risks, and coordinate treatment interventions., • Assist clients in accessing primary care, specialty medical services, and preventive healthcare., • Support crisis intervention and participate in on-call rotation as required by the FACT model., • Document all nursing services in accordance with agency policies, regulatory requirements, and Medicaid guidelines., • Coordinate lab work, follow-up on results, and communicate findings to the psychiatric provider and team., • Provide health education on topics such as nutrition, chronic disease management, infection prevention, and wellness practices., • Assist in developing and implementing individualized service plans and treatment goals., • Monitor client progress related to medical and psychiatric stability, updating plans as needed., • Support the team in identifying and responding to emerging medical or psychiatric concerns., • Licensed Registered Nurse (RN) in the state; BSN preferred., • Experience in behavioral health, community mental health, or psychiatric nursing is strongly preferred., • Knowledge of psychotropic medications, mental health diagnoses, and co-occurring disorders., • Comfort working in the field, including community-based outreach and home visits., • Strong communication, assessment, and crisis management skills., • Ability to work collaboratively in a multidisciplinary team environment. Successful completion of background check including Justice Center and New York State Office of Children and Family Services Statewide Central Register Database Check.
