Job Description MLTC – Field Operations Specialist (Immediate Opportunity) Location: Bronx, NY (4 days field visit and 1 day office) Compensation: $24.03 – $26.44 per hour Job Type: Full Time Job Description A Managed Long Term Care (MLTC) program is seeking a Field Operations Specialist to support care management teams through in-home visits, service coordination, and administrative support. This role plays a key part in ensuring older adults and individuals with disabilities receive timely, high-quality services that help them remain safely in their homes. Key Responsibilities: • Travels to members’ homes and conducts in-home care management visits, • Communicates effectively with members/caregivers and Plan Team (clinical and non-clinical), • Accurately documentations and uses the electronic medical record, • Schedules care management visits timely and effectively., • Submits timely and accurately for travel reimbursement expenses, • High School Diploma (or General Education Degree (GED) required. BA/BS recommended., • 2-3 years in a managed care or home care setting. Valid NYS driver’s license and the ability to visit members in their home., • Must be able to read, write, and speak English.
