As the Registered Nurse in Home Health you will provide and direct provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies. You will also coordinate total plan of care with other health care professionals involved in care and helps to achieve and maintain continuity of patient care by planning and exchanging information with physician, agency personnel, patient, family, and community resources. Primary Responsibilities: • Clinical Competence, • Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other health care professionals, • Supervises care provided by home health aides and licensed practical/vocational nurses, provides instruction, and assigns tasks according to State and federal regulations, • Provides required supervisory visits, • Documentation and Care Delivery, • Provides high-quality clinical services within the scope of practice and infection control standards, in accordance with the plan of care, and in coordination with other health care team members, • Completes comprehensive assessments (OASIS) including medication reconciliation accurately and timely, • Documents patient visits per policy and payer requirements, and syncs timely per LHC policy, • Quality, • Makes initial and/or comprehensive nursing evaluation visits, ensures patients meet home health eligibility and medical necessity guidelines, determines primary focus of care, develops the plan of care within State guidelines with the physician, and submits accurate documentation, • Communicates relevant information timely and effectively with appropriate agency staff, including patient care issues, visit assignments, schedule changes, orders, OASIS data sets, coding requests, and coordination with other clinicians, • Communicates timely and effectively with physicians, patients, and family members to ensure quality care and service excellence, • Teamwork, • Takes direction from Clinical Director and Executive Director professionally and completes assigned tasks timely, including required learning, • Assists in the orientation of new agency personnel and serves as a preceptor to other staff and students, • Actively participates in survey/survey readiness activities and performance improvement plans, works to reduce unnecessary patient hospitalizations, improve patient safety, and implements processes and best practices to ensure positive patient outcomes, • Participates in on-call and weekend rotation as needed to meet patient needs, • Adheres to and participates in the agency's utilization management model You'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in. Job Description The In-House Paralegal provides essential, high-level legal and administrative support to the General Counsel and the Legal Department team, ensuring the hospital operates in full compliance with all applicable laws and regulations. Summarize findings in clear, concise memoranda for the General Counsel. * Litigation and Claims Support: Assist with managing litigation matters, including coordinating with outside counsel, gathering and organizing documents for discovery, preparing subpoenas, and placing litigation holds on records. * Legal File Organization: Maintain and manage all physical and electronic legal files, corporate records, and litigation documentation to ensure accurate, accessible, and up-to-date archives. Compliance & Regulatory * Regulatory Filings: Assist with the preparation and tracking of various regulatory filings, license applications, and periodic reports required by federal and state agencies. * Policy Review: Review and assist in the drafting or revision of internal hospital policies and procedures to ensure compliance with legal and regulatory changes. * Subpoena and Records: Process and coordinate responses to subpoenas, public information requests, and medical record requests, ensuring strict adherence to privacy laws. Administrative & General Support * Board/Governance Support: Assist with corporate governance matters, including preparing and organizing materials for Board of Directors and committee meetings, and maintaining corporate minute books. * Communication: Serve as a professional liaison between the Legal Department and internal hospital departments (e.g., Risk Management, Compliance, HR, Finance) and external parties (e.g., outside counsel, government agencies). * Project Management: Proactively manage departmental projects, track deadlines, and follow up on outstanding legal matters as directed by the General Counsel. QualificationsRequired Experience and Education * Minimum of three (3) years of experience as a Paralegal in a corporate legal department or law firm setting. * Paralegal certificate from an ABA-approved program or an Associate/Bachelor's degree in a relevant field. Essential Duties & Responsibilities: Personal Trainers are responsible for performing the following activities for the club: Service and Train Clients [70% of time] Create an outstanding initial personal training experience for introductory package clients. Prepare and deliver comprehensive fitness programs based on clients goals that include appropriate exercise selection, nutritional programming and teaching a fitness lifestyle. Inform clients of the fitness tools available to assist them in achieving their goals. Set expectations and hold clients accountable to their comprehensive fitness programs through goal setting, follow up discussions, and tracking progress. Demonstrate safe and proper exercise technique to clients. Service Members [20% of time] Instruct members on proper use of club equipment and exercise techniques. Assist, at the club managements request, in any member service activities (e.g., fitness seminars, boot camps, body fat tables, supplement/nutritional product demos etc.). Understand all aspects and benefits of the Gameplan and deliver a positive fitness experience to members and guests. Help with racking weights and assisting in maintaining a neat, organized and clean club. Administration/Misc. [10% of time] Schedule all personal training sessions, other appointments, and administration time using company systems

The Executive Director is accountable for guiding operations to achieve key performance indicator goals set by the board of directors for the WelbeHealth program. Through effective oversight and leadership, the Executive Director ensures that the program delivers high-quality care to WelbeHealth participants and remains compliant with all applicable state and federal regulations. The Executive Director facilitates collaborative relationships across WelbeHealth and with stakeholders, and manages a high-functioning leadership team that successfully operationalizes WelbeHealth values, team culture, and mission. This role is different because the Executive Director at WelbeHealth: • Plays a key role in launching WelbeHealth's first center in New Jersey, • Medical insurance coverage (Medical, Dental, Vision), • Work/life balance - We mean it! 17 days of personal time off (PTO), 12 holidays observed annually, and 6 sick days, • 401K savings + match, • Comprehensive compensation package including base pay and bonus, • Manage direct reports including hiring, training, supervising, and mentoring, ensuring that all staff are assigned duties based on their education, training, competencies, and job descriptions, as well as actively monitor team member engagement and support organizational health practices to ensure exceptional culture, • Participate in the quality assurance program for participant care and staff performance, • Partner with finance leadership to develop, maintain, and operate within a program budget, including hiring plans, to achieve operational goals, • Build strong relationships with key external stakeholders such as community service providers, regulators, legislators, and associations, as well as establishing and maintaining communication with ACF staff and services, support services and community resources, and participants, • Oversee the development of sound data collection methods and maintenance of secure and accurate medical records, • Ensure the development, implementation, and enforcement of policies and procedures, including participant rights, • Bachelor's degree in healthcare administration, public health, social welfare, or gerontology, or relevant field experience in a managerial, administrative, or supervisor position in a health care facility, • Each year of full-time, or full-time equivalent, administrative or supervisory experience or training in a healthcare facility may be substituted for each year of the degree requirement - four (4) years of such experience may be used to satisfy the degree requirement, • Minimum of five (5) years of experience developing and managing highly successful programs for vulnerable seniors, • Minimum of three (3) years of supervisory experience in a health or social services program, with experience managing a senior care program preferred, • Demonstrated ability to mentor and develop other leaders and care team members, • Current knowledge of chronic care, geriatric issues, and best practices, • Track record of driving initiatives that lead to improved patient experience Salary/Wage base range for this role is $182,160 - $255,454 / year + Bonus.
