ABOUT THE JOB PATH's CalAIM program provides services to participants receiving services through managed care plans. Through these partnerships, staff provide support to eligible Medi-Cal beneficiaries experiencing homelessness, including chronic homelessness, and who have at least one complex physical, behavioral or developmental health need with the goal of improving health outcomes and/or decreasing utilization of high-cost services. The Case Manager role's main function is to assist clients in breaking the cycle of homelessness by utilizing a "whatever it takes" approach. This position supports clients through all phases of homelessness, including but not limited to: on the street, interim housing, transitional housing, and permanent housing. Position Responsibilities include: • Provides referrals, linkages, information, and support to resources that help participants to achieve their ISP goals, • Completes program specific assessments (e.g. VI-SPDAT), • Evaluates strengths and challenges to addressing short term and long-term goals, conducts a 90-day review with the participant after initial intake, • Analytical Skills and Attention to Detail: Gather and interpret data, identifying trends, patterns, and insights to inform decision-making. Ability to consistently produce high-quality work, ensuring accuracy and precision in all tasks., • Collaboration and Teamwork: Work effectively with others, seeking out and building positive working relationships with colleagues, participants, and stakeholders.
