City Sanctuary Facility Residential Aide Residential Aides provide ongoing support to all onsite services. Job Description Astor’s Prevention Services/Family Connections is a time-limited, evidence-based model that utilizes a home-based approach to address both the concrete and clinical needs of the family. Prevention services utilize comprehensive family assessment tools to identify behaviors related to risk and to develop Core Outcomes that address the identified child welfare concerns within a family. The Case Planner provides in-home counseling services to families in need of support in developing skill and competencies that will enable them to keep their children in the home and out of placement. The Case Planner will maintain a caseload of 12-14 families and provide change focused interventions with families to improve parental functioning, address life stressors, and improve overall familial functioning. This is happen over a course of 6 to 12 month length of service and a minimum of 2 face to face contacts with each family per month. The Department is comprised of 70+ healthcare professionals who are responsible for providing re-entry services to individuals detained in the NYC jail system. The Social Work Department supports patients’ rights and criminal-legal advocacy efforts in collaboration with patients’ criminal defense attorneys, Alternative to Incarceration (ATI) programs, and other post-release supervision providers. Additionally, our Social Workers coordinate closely with the New York State Office of Mental Health (OMH) and the NYC Department of Health & Mental Health (DOHMH) to provide robust services to our most vulnerable patients. This position reports to the Associate Director of Social Work Reentry and is responsible for collaborating with the Mental Health Service on assessing patients’ appropriate level of care post-release.
