Responsibilities: • Manages a caseload of clients living with chronic or complex health conditions and/or socio-economic vulnerabilities (e.g., chronic conditions driven by social determinants of health, Hepatitis C, HIV, etc.)., • Identifies and advocates for clients engaged in care within one or more of Philadelphia FIGHT's Community Health Centers sites related to shelter/housing, utility assistance, food assistance, clothing banks, medication assistance programs, substance use and/or behavioral services, etc., • Assesses un- and under-insured clients' eligibility for Pennsylvania public benefits (e.g., Medicaid, MAWD, CHIP, SPBP, etc.)., • Maintain regular contact with clients, guided by program requirements, client needs, and care goals., • Conduct biopsychosocial assessments and develop individualized, goal-oriented, and strengths-based service care plans at intake; reviews and updates quarterly in collaboration with the care team., • Provides benefits assistance and enrollment for clients via COMPASS., • Follows up on benefit applications and advocate on behalf of clients with the County Assistance Office., • Provides trauma-informed therapeutic support, including crisis interventions in collaboration with behavioral health and medical providers., • Maintains client files by accurately and timely documenting all client interactions medical records to the electronic medical record system, including benefit applications, appointment adherence, medical treatment plans, medication adherence, and any subsequent follow up., • Identifies and advocates for population-specific support services for those who need gender-affirming care, immigrants, clients who are blind, deaf/hard of hearing, have cognitive abilities, etc., • Supports clients at medical appointments or during home visits as needed to ensure continuity of care., • Participates in on-call rotation during regular office hours., • Serves as a mandated reporter for minor patients in cases where harm is perceived or evident., • Supports department operations by providing cross-coverage for Health Access Services staff (e.g., Medical Case Managers, Benefits Access Navigators, Assistant Case Managers, receptionists, etc.)., • Abides by all compliance standards and operational activities, including but not limited to, completing training, attending all required staff meetings, supervision meetings, participating in events, etc., • Assists with special projects, organizational events, staff coverage, etc., when needed., • Other duties as assigned to fulfill FIGHT's mission. Job Description CORA Services is a dynamic and growing not for profit organization in Philadelphia. We have immediate opportunities for Truancy Case Managers to join our Community Services Division. These positions are full-time and require travel within assigned areas of the city Valid driver’s license required Minimum starting salary - $21.20/hour (approximately $44,100/yr) Work schedule may include occasional evenings All CORA team members are expected to uphold the mission, vision and values of CORA Services which includes valuing all individuals and supporting each individual’s potential for growth In addition, as a Truancy Case Manager your duties will include: • Engaging clients (students), families and other relevant individuals in a collaborative relationship that will enhance the potential for the client’s completion of service with positive outcomes; caseload size will vary, • Working collaboratively with families, schools and court representatives in the development and implementation of service plans to facilitate regular school attendance; providing advocacy services in community settings most conducive to achieving desired goals with clients, including but not limited to clients’ homes, schools, court, recreation centers and treatment facilities, • Maintaining awareness about the availability of community resources and facilitating client/family access to appropriate resources and services necessary to support and supplement service goals, • Collecting/monitoring required school data and client satisfaction materials in the prescribed format; communicating with DHS and the Court and, within privacy rights protections, with school personnel and other service providers regarding service status and outcomes for RTC-involved students and families., • Accompanying and assisting children and families through Truancy Court proceedings; preparing and submitting, in a timely manner, all court mandated documentation related to each client’s case, and statistical reports as required, • Advocating with and for clients for access to appropriate level of care placements, • Adhering to Agency and regulatory entity guidelines concerning caseload requirements, service delivery, accountability and confidentiality, • Participating in required interdisciplinary functions and meetings, • Working productively and collaboratively with other members of the Truancy Case Management team and other CORA team members, • Bachelor’s degree in social work, human services or related discipline, • Demonstrated case management experience, • Knowledge of substances of abuse and their effects preferred, • Valid driver’s license required and driving record must meet CORA standards, • Outstanding communication and interpersonal skills; ability to establish and maintain positive, productive working relationships internally and with external partners (e.g., court & school personnel, families, community partners, etc.), • Ability to effectively manage assigned caseload variability, • Medical, dental and vision coverages with a competitive company premium contribution (eligible 1st day of the month following date of hire), • Company paid life/AD&D and LTD coverages, • Supplemental voluntary benefits including STD, accident, critical illness, etc., • 403b retirement plan with generous company contribution after one year of service, • Paid time off + paid agency holidays, • Employee Assistant Program, • Family Planning Benefits (including cash benefit to assist with IVF, egg freezing, adoption, or surrogacy)
