Job DescriptionAbout Us Homebridge serves a high risk, high needs, complex-diagnosed population with a combination of domestic, personal, and paramedical homecare functions funded through IHSS. Our work allows an often-underserved population to live safely in their community. Homebridge offers a dynamic, fast-paced work environment with 420+ unionized field workers and 100+ internal office staff. Position Summary Under the direction of the Program Manager, the IHCG Supervisor will be responsible for coordinating in-home care services for older adults and individuals with disabilities who require assistance to continue living independently in the community. This position works collaboratively with property management, supportive services, hospitals, and other organizations to assist and support clients in attaining their individualized goals. The IHCG Supervisor will directly support clients by addressing social drivers of health, completing whole-person assessments, and creating person-centered care plans, while supervising a team of IHCG Caregivers who provide domestic, personal, and paramedical care. The IHCG Supervisor oversees scheduling of all client home care services, supervises and supports IHCG Caregivers, and ensures clients receive high-quality, consistent care in compliance with Homebridge’s contract provisions, policies, and procedures. They will help clients manage their care through Enhanced Care Management (ECM) and other strategies, assisting them to improve health and well-being. This role plays a critical part in helping older adults and people with disabilities remain safely living at home. Essential Job Functions Service Coordination and Scheduling • Maintain a caseload of clients and supervise IHCG Caregivers., • Build positive collaborative relationships with clients, IHCG Caregivers, onsite services, roving case management teams, and health care teams., • Conduct whole-person assessments in clients’ homes according to established timelines., • Create individualized person-centered Care Plans for each client, including regular home care schedules and defined tasks., • Provide care management services based on assessments; conduct home visits at least quarterly., • Communicate all relevant client and IHCG Caregiver information to the Program Manager., • Complete all charting, scheduling, and documentation weekly in accordance with Homebridge protocols., • Attend Program Department and other scheduled meetings., • Meet requirements of Enhanced Care Management program for qualifying clients and refer others to appropriate internal and external programs., • Comply with all contract requirements., • Maintain client confidentiality at all times per HIPAA standards. Supervision and Support • Work closely with IHCG Caregivers to best serve clients, including scheduling to ensure all client hours are met., • Assess and support IHCG Caregiver performance; communicate issues and successes to the Program Manager., • Ensure IHCG Caregivers are well-trained and able to perform assigned duties., • Employ a “Coach Approach” in working with IHCG Caregivers; conduct regular supervision meetings., • Participate in screening, interviewing, and recommending IHCG Caregivers for hire., • Meet IHCG Caregivers in clients’ homes to provide support., • Respond promptly to calls from IHCG Caregivers regarding service delivery., • Monitor IHCG Caregiver schedules to maximize time with clients., • Ensure IHCG Caregivers understand each client’s Care Plan., • Occasionally assist clients with basic needs in IHCG Caregiver absence or during home visits., • Document all progress notes and activities in electronic client files. Knowledge, Skills, and Abilities • Commitment to Mission: Dedicated to delivering high-quality care services., • Organizational Skills: Strong time management and task prioritization., • Communication Skills: Excellent verbal and written communication, including coaching and documentation., • Interpersonal Skills: Ability to build rapport with providers, clients, families, and team members., • Analytical Skills: Skilled in conducting assessments and analyzing client needs., • Technical Proficiency: Proficient in Microsoft Office Suite and Medsys; able to learn new software quickly., • Problem-Solving Skills: Proactive approach to addressing client needs and program challenges., • Flexibility and Team Support: Willingness to contribute at various levels to ensure program success. Education and Experience • Associate’s degree in Social Work, Health Care Administration, or a related field preferred., • Bachelor's degree is highly preferred., • Candidates with 3+ years of relevant experience in home care services and client assessment are encouraged to apply., • Lived experience as a vulnerable community member will be considered valuable and relevant experience. Work Environment This position is hybrid-remote and community-based in Los Angeles, CA. While in-office, you will work at a desk in a temperature-controlled office, in modular office space or an individual office. The noise level in the work environment is usually moderate. Some travel: must be able to travel on foot, via public transportation or personal vehicle to all parts of Los Angeles for performance of job duties. May occasionally be exposed to outside weather conditions while traveling to clients. Client homes may include cigarette smoke, pets and/or pet dander. Some exposure to physical risk. Some exposure to client apartments, rooms, homes, and other venues so that services can be provided. May include contact with clients with mental health and/or personal hygiene issue.
