Care Team Nurse Practitioner DEPARTMENT: Medical SUPERVISOR: Site Medical Director POSITION: Care Team Nurse Practitioner STATUS: Exempt SUMMARY: The Care Team Nurse Practitioner (CTNP) cares for patients by performing specified visits and supporting the overall function within a care team. The CTNP supports the care team function by caring for patients through responding to medication refills, addressing lab results, completing forms, requesting prior authorizations, and other patient care needs between visits with their primary care provider (PCP). The CTNP cares for patients with acute complaints as identified by the Care Team Registered Nurses and/or who present as walk-ins to the clinic; patients with abnormal lab results who would benefit from an in-person or telehealth visit to explain results; and/or patients with other requests, such as medication refills or form requests, that are best addressed within the context of a visit. DUTIES AND RESPONSIBILITIES Essentials ● At work or staff meeting on time. Communicates with the Site Medical Director if more than 10 minutes late. ● Completes documentation of clinic visits within 24 hours of the patient visit. ● Checks and responds promptly to urgent care-related calls and messages. ● Maintains credentials including license, DEA, and continuing education. ● Demonstrates lifestyle consistent with biblical standards ● Takes ownership of and attains personal patient volume targets. Clinical Team Function ● Addresses refills according to EHC guidelines (within 2 business days, checking if patient has been seen in the past year, making sure appropriate monitoring of medication is up-to-date, etc) ● Completes forms according to EHC guidelines (within 5 working days, with appropriate medical diagnoses and supporting information, advising the need for an appointment if needed, etc.) ● Responds to triage messages and questions thoroughly and concisely, providing appropriate guidance and disposition, asking clarifying questions, when needed. Responds to “important” and “urgent” messages in a timely manner according to EHC policy. ● Responds to lab results with appropriate disposition, signing off on normal lab results, making plans for abnormal results (via back desk message, triage message, telehealth appointment, or flag to PCP), and communicating urgent results in a timely manner. ● Completes prior authorizations and DME forms thoroughly, by looking through the patients’ charts and providing sufficient information for approval. Requests appointments when more information is needed. ● Works to accommodate walk-in and squeeze-in pts. Takes share of ownership of all EHC clinical activities, including directing management of unscheduled patient needs. Adjusts pace of seeing pts to clinical demand. ● Responds to Rapid Responses and Code Blues, playing an active role in the team’s functioning during an emergency. ● Arrives at patient sessions on time in AM and PM, prepared with appropriate items. ● Uses AIDET in daily hub routine. ● Communicates well with support staff, giving them needed details and instructions, so that they know provider expectations. ● Teaches support staff in order to expand/improve their role (pt educational tasks, medication instructions, diabetes topics, etc. ● Empowers patients by encouraging them to be the primary agent in seeking wellness, encouraging them to make own appts and seek solutions to improve own health. ● Sets good boundaries with patients (plans visit agenda, defers issues to subsequent appts when necessary, polite and compassionate yet firm). ● Prescribes controlled medications according to established guidelines, utilizing appropriate accountability measures. ● Completes charting and referrals as sees each patient as much as possible. Job Description The Opioid Epidemic is a public health crisis with a highly effective but underutilized clinical intervention - millions of Americans are physically dependent on Opioids but only 10% of those likely to have OUD actually access treatment. Bicycle Health addresses this gap by maximizing accessibility, affordability, and overall quality of care by enabling highly qualified clinicians to reach patients broadly and efficiently through our online platform. We're looking for Nurse Practitioners and Physician Assistants to join us full-time in a 40-hour-per-week, W-2 role with a schedule designed to offer both structure and flexibility. We are not currently hiring 1099 or part time providers. The Provider position at Bicycle Health is a remote full-time addiction medicine clinical position with responsibility for the continuity care of a panel of adult patients under care primarily for opioid use disorder. The Nurse Practitioner or Physician Assistant will assess new patients presenting for telemedicine-based care of opioid use disorder, determine appropriateness of enrollment in our care setting, and manage their continuity OUD treatment from initial induction through the duration of their care. The Nurse Practitioner also addresses co-occurring mental health conditions directly when appropriate, and also via referral and care-coordination with outside providers. The NP/PA participates actively in regular clinical team meetings and quality improvement activities, collaborates with our program physicians and therapists, and reports to the regional medical director. What You Can Expect: ✅ One weekend day is required– but you'll have a predictable schedule that works for you. ✅ Choose your weekdays - You have the flexibility to select which weekdays you work, with the expectation that your schedule remains consistent each week. ✅ 8-hour shifts - We are currently only considering 8-hour shifts ✅ Stable, full time employment - We are currently only hiring for full-time, W-2 employees.
