Trinity Financial has developed and CAMBA will provide on-site support services at Van Dyke III, a 180 unit supportive/affordable housing development for individuals and families. Van Dyke III will be located on the campus of Van Dyke Houses, a NYCHA-owned public housing development in the Brownsville neighborhood in Brooklyn. The Van Dyke facility will provide 54 units for New York/New York III formerly homeless families under NYC 15/15. The remaining 126 units will provide housing for low-income households, including a live-in superintendent. CAMBA will have a full on-site office suite and offer case management to promote healthy living, connection to medical, mental health and substance treatment, and assisting tenants to access and maintain public benefits; computer trainings and resume workshops within the on-site computer library; financial literacy programs; access and referral services to other CAMBA programs and local organizations and service providers; community-building activities to help integrate the tenants; and additional supports as needed to help tenants succeed and remain stably housed. Position: Bilingual Case Manager Reports To: Program Manager Location: 405 Dumont Avenue, Brooklyn, NY 11207 What The Bilingual Case Manager Does: • Review all documentation establishing tenants' eligibility for program and make file copies., • Create and maintain tenant files., • Conduct initial intake and assessment of tenants and tenants' families' needs and periodic re-assessments., • In collaboration with tenants, prepare initial and periodic revisions of independent living plans including short-term and long-term tenant goals., • Assist tenants in attaining their goals by identifying and locating community resources for tenants and by making referrals to appropriate services both within and outside CAMBA., • Work with tenants to break through barriers to tenant goals and to assist tenants in advocating for themselves and in moving toward self-sufficiency., • Recommend and implement strategies to persuade tenants to participate more fully in this process., • Monitor tenants' progress toward their goals via regularly scheduled telephone contact and/or face-to-face visits, and document via progress notes., • Participate in case conferences., • Follow-up with tenants and with referral organizations regarding tenant contact and progress with referral organization., • Provide all required information for weekly/monthly/quarterly/annual reports., • May recommend closing of cases in which tenants have: (a) achieved primary goals and have maintained stability for a period of months; or, (b) have not demonstrated a willingness to participate in the process (lost-to-service); or, (c) have become ineligible for services (e.g. moved out of building)., • May act as tenant liaison/tenant advocate with outside organizations regarding such matters as education, healthcare, housing, legal issues, etc., • May schedule appointments for tenant with referral organizations., • May escort tenants to appointments (educational, medical, social service, etc.), • May assist tenants in completing applications for benefits and entitlements, and may process applications on tenants' behalf., • May follow-up with tenants for a period of time after successful completion of their primary goals to assure tenant stability., • May evaluate actual living conditions of tenants through home visits., • May prepare marketing materials for the program., • May prescreen tenants over the telephone for eligibility and may schedule intake appointments., • Must sign NYC DOH/MH attestation stating that candidate has not been the subject of a founded child abuse report., • Flexible hours may be needed occasionally to provide shift coverage until 8 pm. Homeless (often mentally-ill and substance-abusing) men and women are provided with comprehensive services to stabilize their condition in order to successfully transition into to permanent and/or supported housing. CAMBA’s Atlantic House Men’s Shelter is a 200 bed men’s MICA shelter located in the Brownsville section of Brooklyn. The majority of these clients are dually diagnosed with mental illness and substance abuse. JOB SUMMARY: As a key member of the SkillSpring (f/k/a GCD) department, the Workforce Programs Case Coordinator will engage with current participants in the young adult and high school programs in an effort to prepare for and secure employment. Job Summary: The Case Planner provides comprehensive case management services for all youth who reside in their respective Site i.e. Non-Secure Placement or Limited-Secure Placement facility and their families. The Case Planner should be committed to a service philosophy and objectives that is youth-centered and outcome focused. The position is demanding and highly interactive with the youth and their families, staff and community. Case Planners must be comfortable working with youth and families in the context of the facility, the office and in the community of origin. The Case Planner will maintain a caseload of 15 youth both in the facility and in the community on aftercare. The role of the Case Planner is to streamline services and service plan goals, guide the discharge readiness planning beginning once youth are placed, and follow through with the youth/family in the application of the service plan in the community. Case management responsibilities include all necessary tasks to complete case file reviews, case planning, family engagement, and group service delivery. Case Planner is responsible for developing and maintaining meaningful contact with youth, their families, and service providers for medical, mental health, social, educational, and other related service needs. The service plan will be completed in collaboration with the team members, PPS workers and with the youth and his/her adult resources in the Family Team Conference. · Work with the case planning team (Education Specialist, and Support staff), the youth, the family and PPS and any other service provider to develop a discharge plan as part of the service plan. · Schedule family visits within 24 hours of admission to the agency and ensure all parties are aware of any scheduled visits. · Collaborate with the Program Directors to Supervise family visits/phone calls including interaction between youth and family as needed. · Engage the family members throughout the assessment process and complete FASPs, SPRs, Plan Amendments, court reports and other case related assessments as scheduled. · Responsible for meeting required ACS completion dates for accurate progress notes, FASPs, visits, court reports and any other documentation required. · Document all progress notes in Connections within 5 calendar days. · Implements creative strategies for involving family members in the service planning process. · Monthly home visits to engage and assess the family in collaboration with ACS. In addition, complete initial and discharge home assessments. · Works collaboratively with interdisciplinary team and other program/division/support staff. Job Description POSITION SUMMARY City Parks Learn has operated interdisciplinary environmental education programs in schools and parks since 1992. Eco-Innovators is a program for 9th - 12th graders that gives students access to environmental STEM learning and introduces them to NYC infrastructure and leaders as they work to invent creative projects that push the environmental resiliency movement forward in their own communities. Students make real change as they explore urban infrastructure and advocacy tools and learn techniques to help solve challenging environmental issues, including expanding green spaces, increasing recycling and reducing waste, and improving energy efficiency. Reporting to the Senior Manager of STEM Pathways, the successful candidate will have a minimum of two years of experience in delivering instruction to and working with adolescents in a project-based, out-of-school time setting. Reporting to the Community Partnerships Coordinator, the Youth Engagement Specialist will play a key role in both developing and overseeing youth programming at Amsterdam Houses and supporting the Justice Center’s broader community engagement initiatives across the borough of Manhattan. Position: Bilingual (Spanish) Client Advocate Reports To: Assistant Program Manager Location: 1259 Flatbush Ave. What The Client Advocate Does: • Plan, coordinate and facilitate social/peer support events, including group facilitation for clients., • Prepare marketing materials for the program., • Conduct outreach activities at high-risk venues to market the program in order to recruit new clients and reengage former clients., • Develop action plans with clients enrolled in navigation or support services, • Follow-up with clients and with referral organizations regarding client contact and progress with referral organization, • In collaboration with program staff, work with clients to break through barriers to client goals and to assist clients in advocating for themselves and in moving toward self-sufficiency., • Contact clients via telephone, mail and/or face-to-face home and office visits to ensure adherence to program, and document via progress notes., • Escort clients to appointments (educational, medical, social service, etc.)., • Provide all required information for weekly/monthly/quarterly/annual reports., • Create and maintain client files and make copies of all client documents., • Prescreen clients over the telephone for eligibility and may schedule intake appointments., • Assist clients in completing all CAMBA intake applications and forms., • Schedule appointments for client with referral organizations., • Input client data and client progress information into automated database., • Two (2) years of relevant experience, preferably in a mid-sized (250+ employees) organization., • Good written & verbal communication., • Computer literacy in Microsoft Office Suite, • Knowledge about, understanding of, and ability to work closely with, persons with HIV/AIDS and related issues., • Experience and/or comfort conducting online outreach via social media and dating/ ”hook-up” apps and websites. • Bi-lingual English/Spanish preferred. To assist us in achieving this mission, NAC is currently seeking a talented Case Associate to join the Specialized Foster Care Program. The team in the foster care department works with children, families and foster families to ensure safety, permanency and well-being for youth in foster care.
