Our newly formed Physical Health Team in Buckinghamshire has an opportunity to welcome a new member. We support four Community Mental Health Teams (CMHTs) to address the physical health needs of patients with Serious Mental Illness (SMI). Our services include specialised Clozapine and Lithium clinics, annual cardiometabolic assessments, and high dose Antipsychotic monitoring. We also provide education, advice, and support to Keyworkers. Physical health has been a focus within our directorate for many years. We are currently reviewing our processes to support teams more efficiently. You will be involved in delivering our established Clozapine and Lithium clinics and conducting physical health assessments for a variety of patients. We require you to have excellent communication skills and the ability to build rapport with patients and teams. Education is central to our work, and we expect you to have the knowledge and confidence to provide healthy living advice and to signpost or refer patients to appropriate support services. As part of our small but dedicated team, you should have an interest in physical health and a passion for providing the best care to those with mental illness, alongside a good understanding of mental health conditions and risk assessment. Skills in phlebotomy, ECGs, and Point of Care Testing are important, and we can support you in developing these skills. You will work directly with a skilled support worker and be supported by a Clinical Lead. Oxford Health is a great place to work.
