Support service users to maximise their potential, promote independence, and maintain their physical, social, and emotional wellbeing. As a keyworker, you will contribute to individual care plans, support service users to achieve agreed goals, and ensure their mental health needs are met. Responsibilities: • Meet the varying needs of service users and monitor and evaluate activity programmes., • Organise and participate in day trips, hobbies, group activities, closed groups, and semi-structured activities., • Introduce creative and engaging ways to interact with and support service users., • Maintain accurate daily records and keep the Service Manager informed of service users’ progress., • Contribute to the development of individualised, achievable, person-centred plans., • Participate in the formal induction programme, ongoing training, staff development, supervision, and appraisals., • Attend and contribute to regular staff meetings., • Work collaboratively as part of a supportive team., • This is an active and physically demanding role, supporting individuals to live fulfilling and independent lives within their home and community. Physical requirements of the role include: • Supporting clients with regular community activities and outings, • Walking for extended periods and accompanying clients on public transport, • Supporting clients with shopping and other everyday activities, • Being able to respond appropriately and promptly in an emergency, • Carrying out household and practical tasks associated with supporting clients, • Remaining active throughout the shift
